Hello,

As we all know Lantus is cumulative and needs to build a shed in order to be utilized. Sometimes Napoleon will have a string of great numbers only to be followed by highs. This could be bounce - but could it also be that he is not getting enough insulin to keep his shed full?

He is on 3u fat BID now but I am concerned to go any higher as he has dipped to 42 on this dose before. I cut him back to 3U after that but it didn't seem to take him out of the blues.

I am just confused at this point. Could it be he still needs more time to adjust to the Lantus? He has been on it since 5/30/10, but on PZI (wedgewood and BCP before that - DX 12/2009).

The shed, or more accurately, depot, is part of the pharmacology of a drug. It takes approx. a week for Lantus to get the depot established. Every time you change a dose, it either adds to or depletes the shed. If you skip a shot or change the time of the shot, it effects the shed. What you are seeing are bounces or a bit of new dose wonkiness (NDW) and not a problem with his shed.

I think Napoleon needs more insulin to get him out of the pinks and reds.
