I am about out of this bottle of insulin and am thinking I should ask the vet if we should change types. Sam's roller coaster is getting to be a bit much for me. Those low nadirs were great but sooooooo steep!

An insulin switch might not be a bad thing. Looks like your guy isn't getting quite the duration he needs. If you do switch to lantus or levemir, I'd go back to 1u BID with the new insulin. They act very differently and can sometimes be more potent for some kitties.

Agree that you and Sam have given this the good fight. I would read both the Lantus and Levemir stickies and check out some of the threads on both groups. I think Levemir seems to be a little more forgiving in terms of testing and monitoring, or maybe it is the people who post on Levemir. Both groups are full of wonderful supportive people and both insulins seem to work for cats.

Levemir doesn't require less testing - it really gives the same type of curve in general. There are a lot of people in the lantus ISG that really push testing and a lot that even test too much imo. They tend to push testing a lot more than other groups for some reason, but it isn't because of the nature of the insulin really. The levemir ISG seems to be more laid back and understanding imo.

If I had to choose between the two, I'd go with Levemir because it seems to be less fragile than lantus and doesn't poop out as quick (you can use the pen/vial longer than lantus). This is just from my experience so far. We have only been using levemir for about a month. We used lantus for about 8 months.

I would be tempted to try a different insulin when your PZI runs out. ECID. Maybe Sam would respond differently (hopefully better) on a different insulin. You've really done a lot to help Sam and I'd want off the roller coaster too. Seems like a different insulin is worth a try.

If it were me I would try a switch. For cats who tend towards big swings from what I have read they often get flatter curves on one of the Ls. If you haven't already, I would recommend posting in both forums and saying you are considering switching, and what do people recommend? I would imagine there may be some cost differences as well as insulin differences, and also a factor is if your vet is comfortable with one of them that might be an easier route than the other one. I think you have given PZI a fair shake and though I think solid regulation may be possible on PZI, it looks to me like an uphill battle to get there.
