Riley Update, Something new wrong...

It's been since mid June that Riley hit remission. He's been doing great.
Until last weekend. He's lost a considerable amount of muscle mass all of the sudden and lost some weight as well... His hind legs lost some muscle mass with having diabetes but since the weekend he can barely walk and he's not jumping up to sit on my lap anymore. When I pick him up his legs twitch like crazy. I also noticed his head twitching a bit. He seems depressed. He's siamese and always talks but he's not talking much since the symptoms started. He's eating and drinking water OK and there is no change in his litter box.
I tested his blood levels today and he's at 120.
I'm wondering if this disease has contributed to his sudden decline and if anyone else has experienced anything like this?
He's almost 16 years old and I'm afraid this is it for the poor guy. I'm planning to bring him to the vet tomorrow but any advice would be great.

Oh, poor Riley! I have no idea but I am praying that it is something that can be taken care of easily.
 
Sudden health changes = go to vet to rule in/out new medical condition.

Loss of weight frequently happens with renal disease and may happen with other serious illness, too. Renal disease allows toxins to build up in the body (uremia), which drops appetite, may make the breath smell like urine, and may result in electrolyte imbalances affecting ability to walk, etc.
