Well, Dax is showing just what some good TLC can do. Today marks a week since Dax and Max entered my home and heart forever. Already, it is looking like we have achieved remission for Dax! His numbers look EXCELLENT currently at 2 1/2 days OTJ. Going to watch closely for at least the next 2 weeks and make sure his numbers stay stable, but so far I'm checking him at the times he would have normally gotten insulin, feeding him, and then checking 40-60 minutes later, and I'm seeing very good trends with his numbers staying the same or dropping slightly from the meal to when I check him again. All it's taken is getting him onto a good diet (currently using Wellness grain free canned foods + mixing it with some raw, trying to get him completely onto raw). I am very pleased to see such great results, I really thought it would be MUCH harder to get him into remission! I shared his chart with my vet today, and she was THRILLED to see such positive results, but said where the diet he was getting before coming here was far from ideal, and higher carb then the Wellness canned foods, she isn't that surprised he has done this well. Fingers crossed for a smooth OTJ trial, and we can stay off the insulin!

OTJ Trial instructions. Please note the section I have bolded. Testing 3-4 hours after food will give you a better idea if the pancreas is kicking in and producing insulin.

Here are the instructions for an OTJ trial:
  • Test at your normal AMPS and PMPS times. Feed multiple small meals throughout the day as much as possible (small meals are less likely to overwhelm a newly functioning pancreas). The morning test is now called the AMBG. The evening test is now called the PMBG.
  • If your cat is green (0-99 mg/L human meter, 0-130 mg/L pet meter, 0-5.5 mmoL) at your normal test times, no need to test further until the next "PS" time, just feed small meals and go about your day. If the numbers are blue (100-199 human meter, 130-230 pet meter, 5.5-11 mmoL), feed a small meal and test again after about 3 hours. Food raises BG's. So if the number is lower 3-4 hours after a meal, then the pancreas is working!
  • After 2 weeks, if everything is looking good, we have a party! And boy, do we party hearty. :cool:

Sometimes, the trial doesn't work the first time and we have to give a little more support by starting the cat with a tiny dose of insulin again. I had to do that with Wink. We simply start the cat back on a tiny dose of insulin to support the pancreas with healing for a bit longer. Our goal is a strong remission and it's better to be safe now then sorry later that you rushed it. With just a little more time we will probably get that strong remission we are looking for.
 
We're now in day 4 of the OTJ trial. His BG was actually a little lower then I would normally have been comfortable with, but with no insulin on board and him acting great, I'm not worried. He is encouraging me to stop testing, but I'm not comfortable doing that, I want to watch his numbers for a couple weeks and make sure this is a real strong remission. You can see how quickly he started into remission - I started to do an OTJ trial and then his numbers went over 100, and I got nervous the upward trend would continue and just have .5u instead of the 1u he was getting, and that dropped his numbers back down and they've stayed down below 100. Him and his brother are both doing amazingly well.
 
Great news! You can look at my spreadsheet and compare to yours, it was very similar. dancing_cat I continued to test every morning during the OJT trial and then backed off to twice a week and now I'm still testing once week. Eventually I'll back off to every two weeks and then monthly I guess. Dusty enjoys the extra attention during the testing so no reason to stop :mrgreen: .

Good to know! I will probably send you a message later, what vet to you use? I have been using Cheyenne West for many years and really like them, but I live on the east side of Las Vegas now, and am trying to find another good cat experienced vet closer to me for emergencies - I'm in a wheelchair so getting to Cheyenne West fast if something goes wrong may not always be possible.
