AlphaTRAK® 2 Blood Glucose Test Strips Lot # 1358646
Dear Valued Customer,
Recently, it has come to our attention that AlphaTRAK® 2 Blood Glucose Test Strips lot 1358646 produces erroneously low blood glucose results when using the gray AlphaTRAK® Blood Glucose Meters. Erroneously low results that are not recognized may pose significant risks to a pet's health.
If customers use the purple AlphaTRAK® 2 Blood Glucose Meter shown below, blood glucose test results are not affected and no further action is needed.

If customers use the gray AlphaTRAK® Blood Glucose Meter shown below, blood glucose test results may be affected and further action is needed.

Our records indicate that you may have received AlphaTRAK® 2 Blood Glucose Test Strips lot 1358646 on or after June 4th, 2013.
If you have the affected test strips in your current stock, please take the following steps:
1.Do not continue to distribute or use the affected test strips lot. Please immediately dispose of the lot in accordance with your local and state disposal regulations. If the lot numbers you currently have in stock are NOT lot 1358646, you may continue to distribute or use these lots. There are no other Abbott Animal Health products impacted by this issue. There have been no reports of injury with this lot.
2.Return the attached form and call Abbott Animal Health Customer Service at 1-888-299-7416 for a replacement order.
