Proper dose?

My kitty has been on Lantus for over 2 years and still isn't regulated. We have reached 5 units twice a day and still his numbers are staying in the 300s and low 400s - so vet recommends increasing to 6 units in the am and 5 units in the pm and recheck again in 10 days. I'm afraid of this high dose and wondered if anybody else has had to reach this high level to try to regulate their cat. Is 11 units daily dose too much? Does it take that long for insulin to work? Thank you
 
Have you done any home testing during this time? How were dose changes made? By whole units (like from 1 to 2 to 3.....or in smaller increments like from 1 to 1.25 to 1.5?

We are very data-driven here and have a spreadsheet we all use to keep track of our numbers. Most of the time, the answers we seek are in those numbers.
 
My girl got to 8.75 units before she turned it around. But I started low, and increased by no more than 0.5 units - most of the time increased by 0.25 units at a time. And I tested several times each day/night so I knew how low the dose was taking her. So I knew she got to that dose safely. It took me about 5-6 months to get there.

Thank you. I’ve kept tracking chart. We started low and slowly increased but lately vet is making big 1 unit increases. We are at 11 units now. Will keep on course and try not to worry

Yes. I do home testing and glucose curves. Have kept chart also since 2018 but he’s still resisting insulin. I just don’t know how much more he’ll need. Thank you
 
You should probably go ahead and ask your vet to pull some blood for testing for high dose conditions like acromegaly (benign pituitary tumor which sends out excess growth hormone) and insulin autoantibodies or IAA (think of it like a allergy to the injected insulin).

The only place that runs those 2 specific tests is Michigan State University so the blood is pulled by your vet and then sent to them for testing.
