Thomas's PPMS is 449. This is the first time it's been below 500. His nadir today was 110. Should I drop his dose? I feel like 3 is too high for 449. He is RAVENOUS.

I agree - better safe than sorry. You're just starting to accumulate data so you don't have a long history of doses and corresponding BGs to guide you. This insulin can also make him drop quickly and go low so you need to be careful.
