Please Help, I cant figure out whats going on

My guy had a bad DKA episode on 12/14/13. We thought for sure we were going to lose him but managed to get him help in time. Since then I've been chasing his numbers all over the place! First the vet have us give him pedialyte for a couple days, I thought once we quit his numbers would go down. I mean the stuff is full of sugar! However they didn't. I called back & asked the vet if he could possibly have an infection since he's had antibiotics for teeth/gums in the past. She says infection doesn't raise BS, HUH???? This was last week so I called on Mon but she wasn't in & I haven't heard back yet. I don't know what to do anymore :sad: And not sure how much I trust her since all she does is try to shove the stupid SD MD food at me, I refuse to buy that!

I tried this last week to keep him at the same dose but that didn't work. I've been giving him IM shots mid cycle a few times on her instructions, she says IM wears off in 4hrs vs SubQ. The IM shot works but as soon as I go back to SubQ he spikes right back up again. Is he getting resistant to this insulin? Could it be infection? Bouncing? I know the insulin is not compromised as it's a new bottle & it does drop it. He gets all Fancy Feast/Friskies Pates 10% or less. Only the beef when low & freeze dried chicken treats. Tonight he's not really eating & being super lazy which worries me. I fed them Turkey (no carb) & he got 1.5U, +4 pm shot he"s Hi. About to check him again & if it's not coming down another IM shot. But this is getting outta control, I need to get him regulated & asap!! Please help!!! I've attached the new chart I started since the DKA weekend. Thanks!

Could you add the insulin to your signature, please? I'm going to have to search fo it because I don't rmember. Found it: its PZI.

I thought there was an unflavored Pedialyte with no sugar? Nope, even unflavored has dextrose. I did see there were some recipes online; perhaps one could be adapted to use gelatin (a protein, not pectin, used in Jello)), rather than dextrose.

I'M shots? Depends on the insulin. Should not be done with Levemir, not Lantus. Could be done with non-depot insulins, but what's the rational?

If you're using ProZinc, PZI, or Humulin, it could work better to shoot every 8 hours (TID) until the condition(s) is(are) resolved. You take the total daily dose, divide by 3 and that is where you start. It will increase your testing and supply use at least 50%.

Next time, I'd tell her that infection can raise glucose levels - it has been documented in cats as well as humans. (OK, maybe tactfull and do it as a reminder, as oposed to an instruction)

That helps, Misty. I can see the ss under the old name.

I think he is really bouncing around. You could try a sliding scale and shoot a different amount for the bounces and then the lower preshots qnd hopefully smooth out the cycle. I'd suggest you get your ss published (we can help) and then post regularly on the PZI forum. We can help with daily dosing and see if we can't get things to even out for him. Here is the forum. Everyone there has experience with your insulin.

Sorry I got those updated so everyone can see & added insulin to the siggy. I have a newborn at home so forgive my brain fog lol. I did get locked out & it took a few days to hear back from admin so I had to start a new account to post.

Yes I was using unflavored but it still has it. She also suggested some water additive for teeth health awhile back. Stupidly trusting her I just used it but when all this happened I looked better at the ingredients & it has glycerin (sugar alcohol) so we've since quit that as well. ohmygod_smile

I have no clue on the IM shots, she just said it gets him down quick & wears off in 4hrs. Preventing him from climbing any higher, 500's mid cycle is just crazy b/c it should be already working by then! I'll give her this it does work but I have no clue about the logic behind it. I'm not hugely familiar with how this insulin actually works other than it's 12hr shed & peaks around 6hrs. I have discussed things I've learned on here with her, she is open to listen but I can tell she doesn't always agree. She does say if it works for him then keep doing it but sometimes I just question her initial instruction. When he 1st got dx'd he went hypo when I brought him home (like 40) & she told me to still give him 3U. Thank god I refused (with help from here) or he would have died! So I question how capable she is to treat this ya know?
