Pip - DX nov 19, new vet switching from Prozinc to Lantus

Hello everyone and their sugar cats :)

My Pip has been on Prozinc for 12 days, original vet prescribed 3ui, I went down to 1 and now am on 1.25.

New vet said with his 18lbs and his current curves he should be starting at 2ui of Lantus tonight.

Should I go straight to 2ui or stick to 1.25 or go to 1.5 perhaps?

And yes I did get the proper u-100 serynges don't you worry :)

When switching from another insulin to Lantus, we take into consideration how they are doing on their existing dose. If you were following Tight Regulation for dosing, and starting new on Lantus, his dose would be 2.0 units. (#kilograms x 0.25). Your spreadsheet says SLGS for dosing on Prozinc, have you considered what you want to do for Lantus? Is he eating all low carb wet food or raw diet? That is one of the prerequisites for following TR. If following SLGS, you could probably start at 1.5 units.

Yes he has be switched to all low carb wet food and I am currently reading on the methods. TR seems to work well but I only ever have 2 days of full curves on weekends because I am back in school for a career change and not allowed to miss much.

I currently do a curve on Saturdays and I've had the last 2 wednesdays off so I got quite a bit of data in a short time.

Can I switch from SLGS to TR and go from 1.25 prozinc straight to 2ui lantus or should I at least do a few days of an in between dose?

TR doesn't require a curve every day, but rather at least 4 tests per day, 2 each in the AM and PM cycle. That PM test can be just before bed, if it's at least 2 hours after the PM shot. It lools like more of the days you've been able to get the required tests in. I would suggest starting Lantus when you can monitor closely, maybe that's the weekend? Some cats have a stronger reaction to an insulin switch.

You aren't locked into a dosing method, life situations can change. You can go straight to 2.0 Lantus from 1.25 PZ if you'd like to try it.

Good question. By +2 and +4 you should have a good idea whether it'll be an active Lantus cycle. On the other hand, due to the depot nature of Lantus, you often need to wait several days for the depot to build and show you action. I've seen the "stronger reaction" happen on both the first and second cycles. If you can possible stay up later tonight if he's diving by +4, or alternatively leave out food in an autofeeder or place he can get it later, then switching tonight allows you to get another cycle to build the depot before going back to school on Monday.

I see you are Montreal, waving from the west coast. I'll likely be up for your later tests tonight. Depending on what time PMPS is of course.

I'm new too, but wanted to welcome you and say you'll probably be very happy in the switch. My Nico was on PZ for a month and not doing well, when my vet decided to switch him over. After the switch, I'd read how much better in general it can be with a cat.
And as fur the TR, some test a LOT in general. You'll also see ss that show every hour, but that will be from those continual glucose monitoring systems. The TR testing intimated me at first too, till Wendy told me what she said above. You don't have to go overboard. Now please don't look at mine as an example, because it had been started by more than 1, that may not is not behaving typical. But before his numbers started changing a lot, I only tested 4-5x many days. And I've only done 1 curve so far.

Did Pip get an ultrasound to see if there's anything up with his digestion? Or bloodwork to determine if he's having pancreatitis? Between Dr. L and the very knowledgeable and available people here, you're in very good hands.

Since I had an ultrasound and bloodwork from 2 weeks ago he said we dont need it right now, just cerenia and lantus and close monitoring and if he keeps vomiting we'll do another ultrasound next week :)

His appetite was back for his dinner about to give his first shot of lantus now.

You format your daily post so that it looks like these.
So it has the date, kitty's name, and then the blood glucose readings, starting with the AMPS (AM preshot)

You can see that Indy had a question, so prefixed the post with the ? icon so people can quickly see there's a question being asked.

There's no hard and fast rule! Some people post every day, some don't. There's a nice sense of community and we like to know how one anothers' kitties are doing, so daily posts seem welcome (I'm new myself, but this is my experience so far)! But there's no pressure to do it either if you have a busy day or are otherwise inclined.

I think it's helpful in early days to keep eyes on your cat's progress just in general, too. Makes answering the questions that much easier.

I have to agree. Even if all you post is the "headline" and don't have any questions or anything to say, it keeps everyone updated on your progress each day and helps when you do have questions or need their advice.

Especially when you're getting started, it's a good idea to post daily.
You might not recognize when it's time to change Pip's dose, for example.
The wise, experienced eyes here know how to assess how your cat is trending.

Right now, Pip will be working on building his insulin depot, and showing us what he wants to do with Lantus, which works differently than ProZinc.

Hungry and snuggly sounds like a great start!
