persistant low bg

hi all, looking for information about low bg. Coltrane was dxd Feb 2020 and has done very well on Lantas, most recently 4 units bid. Yesterday he threw up after eating, just after is morning dose. Threw up again when I tried feeding him an hour later. He does that sometimes, but usually I can get something to stay down. I decided to monitor, and probably wasn't as careful as I should have been, but we've never had this happen before. So I checked on him a couple of hours later and found him very disoriented, classic low bg symptoms, acting drunk, pacing, not making eye contact. I fed him and he ate a full meal, this time keeping it down. I also put a bunch of Karo on his gums. He improved for about a half hour then started worsening again and panting. We rushed him to the emergency vet, where he remains at least until tomorrow.

At the hospital, upon arrival his bg was 70 (that's with the syrup plus a 3 oz can of food in him). They got his BG up to normalish (120) within an hour using IV glucose. He stayed on that drip for a couple hours. At dinnertime it wasn't high enough for them to feel ok about giving insulin but was well in the normal range. We visited him last night and he was engaging in eye-contact and purring and seemed pretty normal. At 6:30 am the vet called and said he went up to 300s and then 450 about 6 am, and had eaten breakfast so they were going to give him 3 units (1 unit less than we had been given). By mid-morning he was down to the 70s - they resumed the glucose drip and by 12:30 pm he was at 50. This was nearing the peak of the Lantas. The hospital ended up being very busy and we didn't speak to them again until just a bit ago. Turns out his BG went up to 156 late afternoon so they stopped the sugar water drip but decided to hold the insulin because he wasn't eating (typical for him when stressed) and BG still too low for their comfort. Then the BG dropped to 70 by 7 pm. That's 12 hours after the insulin dose. They resumed the sugar drip and he's due for another measure about now.

So now the plan is to just try to get him up again, hoping that he gets into the 300s again. If he eats breakfast, they may try 1 unit in the morning, and keep him for the day to do a curve. We'll bring him home around dinnertime. If he doesn't go up they way we want, they will hold the morning insulin and probably send him home with the idea of his regular vet doing an assessment in a couple of days.

Has anyone had difficulties getting low bg up again like this? Any thoughts on this? We're so afraid for him. Prior to this we had no problem, and he was totally asymptomatic for diabetes - no weightloss, no excessive drinking or peeing. What is going on?
 
Lantus is a depot style insulin. As such, the low numbers can last for a long time. With a hypo, it could be as long as 16 hours. I'm glad you caught it and got him to the hospital. Also, cats that have experienced hypo symptoms can be very sensitive to insulin after, so it's a good idea to start low when/if you restart insulin.
