Hey everyone,

Another day, and not much else to report, hopefully this means we are just in the midst of the marathon, and the results will follow in time, baby steps, if you will!

Peeka's +3 last night came in at 191, and I was very excited to see that. +4 was 210, so there is that. AMPS was 375 which is sort of a bummer. I read in one of the stickies that we are trying to stay within the 200s. Not sure what else besides a longer lasting insulin would help get us there for the most part, but the numbers feel somewhat more manageable, epecially knowing where our target range is.

We are about to get into the weekend again which means less PM testing, I am going to try and do some mini curves Saturday or Sunday during the day to try and obtain as much info as possible and see if another increase is warranted for her insulin.

Thank you, to whoever has been keeping up with my somewhat boring updates. I wish we had a good success story to share, or something good going on besides just the daily grind, but I suppose there is a bit of relief in these times where you are chipping away. As everyone says here, it's a marathon, not a sprint.

Allowing for the +/- 20% variance in meter readings, Peter, those numbers are pretty much equal. :)

The first target in the regulation process is to find a dose which avoids the cat becoming hypoglycaemic which also keeps them under the renal threshold. The renal threshold varies between cats and also the reference range varies somewhat between sources. According to Merck's Caninsulin website the renal threshold for cats is BG < 14-16mmol/L // 252-288mg/dL as read on a veterinary meter. The reading for BG levels are lower on human meters than veterinary meters, hence the FDMB target of keeping the cat's BG under 200mg/dL as read on a human meter.

To determine a particular cat's approximate renal threshold you can opt to check urine for glucose in addition to BG monitoring. When a urine test comes up negative for glucose then the readings you've taken in the hours since the previous time the cat had a pee should be under its renal threshold.

Is this something I should look to consider, or is getting the BG levels good enough? I'm hoping to not have to do additional testing, I just got the hang of the ear pricks and am going to start showing my sister how to do them this weekend!

All that said, is there any advice on Peeka's current levels? I know some have said they would have loved numbers in the 200's when they were first starting. My wife will make comments about the PS tests and how high they are, and while I agree, I keep pointing to her nadir and saying we don't want to get too low there. Would love some input on her current levels and their thoughts, especially considering the recent diet change.
 
Is Peeka 100% on low carb food now? If yes, what date did she last get any high carb food (even if it was only a small % of her daily food)?

Urine glucose is an optional extra, not a necessity. ;)


My wife will make comments about the PS tests and how high they are, and while I agree, I keep pointing to her nadir and saying we don't want to get too low there.
You are correct that the dose needs to be set so that it doesn't take BG levels too low at nadir.

The big gap between the PS and nadir BGs you are seeing are typical of cats being treated with Vetsulin/Caninsulin. Here's the graph showing how it works to remind you:

Any chance you can grab some +3s / +4s over the next 3 days, Peter? This is necessary testing. You need to know how low the current Vetsulin dose is taking Peeka before her next dose review is due (7th day on 2.0IU dose).

Armed with that information you'll be able to to start making the small, steady dose adjustments which should gradually nudge Peeka's curve down into healthier numbers.

Pretty sure I can get +3 and +4s, especially on Saturday or Sunday since I'll be around the house during the morning time. I can probably get a +4 tonight as well.
