Ok, I know everyone saids dont freak out, but I am. My cat Cheynne had been in remission since last August 2012. He has been fine. A little stiff here and there but he is 16 years old and has hyperthyroid. I haven't tested him since June. Tested him today since he has been acting a little odd. Was 175. Will retest in a couple hours. When do I know for sure if I need to start giving him insulin again??

How long has he been acting odd? He might have an infection or something else going on that's causing the spike in his sugar. Are you planning on taking him into the vet? Maybe test him for a couple of days to see if his numbers stay consistently above 150. I would also probably test him before he eats and then again a couple of hours after he eats to see if his numbers are going up or coming down. If they come down, that means his pancreas is still working slightly and there's a better chance of remission a second time around or he might not even need insulin to start.

The other week, Maui got sick (could have been pancreatitis, not really sure). Tests taken and nothing definitive. I took her Bg's and it was in the 200's, normal is 70's.
I got concerned that she may need to go back on insulin too. I even had the vet write me scrips, just in case.
Once she started feeling better, her bg's started coming down and are back to normal.
Give it a few days, let her get better and if her bg's don't come back down, then consider the need for insulin. Hopefully, it won't be needed. But nothing wrong with getting prepared and getting the scrips, just don't fill them and spend the money yet.

Has anything changed? Food? Any signs of a UTI? Or bad teeth? or ear infection?

Thanks for all your concern. His blood sugar has come down. Was 95 on Wednesday, but took him to the vet anyway. Pancrese was normal. All blood and urine tests came back today as good. The vet thinks possibly he caught an upper respitory from a kitten I had brought home a couple weeks ago. He has lost his voice 
So he is on antibiotics and I think he feels better today since he was grooming himself and he hasn't been doing that lately. Blood sugar was 115 just now, but after listening to you guys I am not going to panic about that. Thanks again.

Tips to stay OTJ
We say a cat is in remission if the cat can maintain BG levels for 14 days between 40-120 with most of that spent under 100. However, please keep in mind that once a diabetic always a diabetic.
1. Never feed dry - not even treats. If you change wet food types, be 100% sure the new food is also low carb and same low carb % as your current food. Some cats are very carb sensitive and an increase from 3-6% to 8-10% can spike the BG’s. Don’t feed if you aren’t sure!
2. Weigh every 2 weeks to 1 month to watch for weight changes. Too much of a weight gain can cause loss of remission.
3. Measure blood once a week, indefinitely. You want to catch a relapse quickly.
4. No steroids or oral meds with sugar - remind your vet whenever giving you any medication. Always double check.
5. Monitor food intake, peeing and drinking. If increasing, a sign of losing remission.
6. Regular vet checks for infection such as dental , ear or UTI. And get them treated quick!
If he does fall out of remission you need to be more aggressive and resolve issues/ back on insulin as soon as possible as the window for a second remission is tight if any.
