I am writing at the advice of Nicole who was responding to my earlier posting about Sander. I wonder if an advisor out there can help me interpret my data on my cat Sander who was on a continually reduced dose of Lantus for 4 weeks, then OTJ for 10 days, and now is trending upwards in the blue.

My data is in Sander's spreadsheet in my signature. The numbers are irregular because I am new to regular testing and I was also away for a week.

Thanks,

Elisa

Welcome back Elisa.

I read your other thread and Nicole provided you with much of the information from the Tilly Protocol. I want to suggest a bit more homework. I'd encourage you to familiarize yourself with the starred, sticky notes at the top of the Board. There is a modified version of the Tilly Protocol in the Tight Regulation sticky that's a bit easier to read and digest. Here's a guide to the sticky notes:
  • Tight Regulation Protocol: This sticky contains the dosing protocol that we use here. There are also links to the more formal versions -- the Tilly Protocol developed by the counterpart of this group in Germany and the Queensland/Rand protocol developed by Jacqui Rand, DVM and published in one of the top vet journals.
  • New to the Group: Everything you wanted to know about this forum and more. Info on our slang, FAQs, links to sites on feline nutrition and to food charts containing carb counts, how to do a curve and the components to look for, important aspects of diabetes such as ketones, DKA, and neuropathy, and most important, info on hypoglycemia.
  • Handling Lantus: how to get the maximum use from your insulin and what to not do with it!
  • Lantus depot/shed: This is an important concept for understanding how Lantus works.
  • Becoming Data Ready: What data you need in order to be able to work toward remission or tight regulation.

With respect to Sander, you really don't have enough information for me to give you any reliable guidance. The minimum amount of data that we need is your AMPS, PMPS and at least one spot check in both the AM and PM cycles. Most of us collect much more BG test data. You are welcome to look at Gabby's SS (I'm a testaholic, so don't panic) and you may want to look at Nicole's Baby's SS, as well. What I can observe is that we do not reduce doses in 0.5u increments. Most dose changes are made in 0.25u amounts.

With the trend in Sander's numbers, it does look like he may need to be on insulin. I would suggest getting a mini-curve (testing at AMPS, +3, +6 and +9). You are basing decisions on a single number -- usually a pre-shot number. Lantus dosing is based on the nadir.

Please let us know if you have questions. We're happy to help.
