Not so happy Monday for Boris

Previous post for background.

I’ve scheduled a pre-dental appointment for Boris on our first full day in Las Vegas (Monday, 4/20), with his dental procedure set for Thursday, 4/23.

Recently, his numbers have been trending more frequently into the 200s. I’m becoming increasingly concerned that he may need intervention before we make it to the dental. I’m also worried about fasting him for the procedure given his history of DKA.

This morning, he had what I’d describe as an “empty stomach” vomit—just clear, frothy liquid. He was given additional Friskies pâté overnight around 2:15 AM (along with my IBD cat).

Currently, we are:
  • Testing AM and PM before his usual meal times
  • Feeding him between meals as he shows hunger
I haven’t checked ketones recently, but I plan to try to collect a sample today.

How concerned should we be at this point? Would you recommend taking him to a vet now, or continuing to monitor?

We’ll also be at the Grand Canyon this weekend, where veterinary care will be less accessible. For now, we’re in Winslow and could get to Flagstaff, which has more resources, if needed.

The link to his spreadsheet is in my signature. I also graphed his numbers in Excel (I’m not sure how to do that in Sheets), and the overall trend appears to be increasing.

I reviewed the spreadsheet it look ok, for the exception of the main information for Boris, ( see Corky's SS,) you need to add the dark beige portion to the SS,
His BG is increasing, has there been any new changes in his surroundings, recently? diet change? I mee you are feeding Friskies Pates and various flavors, perhaps some of the flavors are higher carbs? below is a Drs approved list the third column contains the carb %Dry Matter Carbs is what we aer looking for
Corky had his first dental on the 8th, this month see SS, I was panicking he is very tightly regulated, I read all the information on the link below, it depends on what time he has his morning shot and feeds, Corky's feeding and shot is at 6:00, he was at the vet for the procedure at 9:00AM, he had his shot he fed, by 1:00 PM he was home, he ate like a champ asap, I was a mess he is 14, healthy as a log, but I was frantic over the anesthesia,, the link below has a chart on feeding , insulin, ask the vet a at what time he will do the procedure if needed, this way you will know if to feed shoot a full dose or partial, let him know the time you shoot and feed, I wash FRESH! I showed my vet all I learned thru this link and asked all kinds of questions. this link is very informative, don't be afraid to ask and take a picture of the information chart on feeding/shooting

Is there a better way to indicate an ongoing thread so that’s easier to see? It’s been over six years since I last posted on FDMB, so I’m sure I’m a bit rusty on best practices.

According to Dr. Pierson’s chart, the Friskies pâtés are all in the 4–6% carb range, so they should be appropriate. That’s what Boris was eating when he went into remission and what he has consistently eaten since. We did allow some high carb dry food to creep back in, but I removed it once his numbers started rising.

Boris’s spreadsheet was originally created in 2019, so you’re correct that some of that information isn’t included. I’ll work on updating it, although there hasn’t been much to add while he’s been in remission.

What I’m mainly looking for is guidance on whether he may need veterinary care before his pre-dental appointment next week, or if numbers trending into the yellow range are acceptable for now given his history of DKA. That’s my biggest concern. Similar to his initial diagnosis, coming out of remission feels like uncharted territory, and I’m unsure what the current recommendations are.

I will ask our computer wizard to see if we can link your 2019 Threads, I don't think you need any pre-care before his dental appointment, I would assume he will give you another appointment if needed for that so you can prepare

I was simply trying to politely ask Corky to review my previous thread, which I linked at the top of this one. Her initial reply seemed to make several incorrect assumptions about Boris’s situation, so I wanted to clarify. That post from Thursday also includes more detail about our atypical living situation, which affects how we schedule appointments and manage our cats’ care—hence why I referenced it here.

We’ll also be traveling to the Grand Canyon this weekend (with our cats), and I want to make sure I’m not being negligent in his care. I’m not sure where the nearest emergency vet is to Grand Canyon National Park, and I’d prefer not to find out the hard way.

i am so so sorry now I got it, it's been a rough day today, my apologies to you, I read the thread, a non-diabetic cats' glucose levels are between 50-120, so after reading that post it would be a good idea to have Boris checked out, ask for a KETONES test,, and it can also be a dental issue, cats are sooo good at hiding illnesses, touch the tips of Boris's ears, if they feel unusually hot, it means there's fever, I learned this from Corky's vet, it may be something as simple as that, Bori's numbers right now are not remission levels, but they are not that bad either but as you say not normal, so whatever it is , you have seen it in the BG level change, by your continuing home testing, good job! Today is my youngest girl's birthday that I have not seen in almost 5 years, I have been keeping busy here, but I lost my train of thought for a minute there, Enjoy Grand Canyon! ✈️ ✈️
 
I just tested him for ketones - still negative.

Due to the upward trend in his BG and our trip this weekend I made an appointment at a local vet tomorrow. I want to make sure there isn't something urgent going on that is causing his issue, and get an answer to my question: When do rising numbers become urgent? At what point should I be concerned that he may need insulin again?
