Hi everyone

After an hypo glicemia episode Noah was not eating
I brought him to the emergency vet Sunday but they didn’t check his ketones just gave him Celenia injection and anti acidity and back home
After 2 days of almost not eating he is not today all day at the vet that confirmed ketoacidosis
He is getting fluid and antibiotic
His blood test were not so bad at all but high white blood cells

tomorrow I need to bring him again to the vet for fluid
I have lots of questions and doubt in my mind

any suggestion from you expert on this forum?
I never manage to regulate his diabetes, he bounce all the time , he is more than an year diabetic so I’m following a reductions just if he go less than 2.2

High white blood cell counts typically means an infection. They have to treat that too. From this post: Ketones, Diabetic Ketoacidosis (DKA), and Blood Ketone Meters
The basic recipe for developing DKA = an insufficient supply of insulin + inappetance + infection OR other systemic stresses.

Plus you will need to get something to help his appetite, an anti nausea medication to give at home every day. Cerenia is OK (good for vomits), but ondansetron is better. You may also need to give an appetite stimulant, given after the anti nausea. Mirataz is a common one and easy to give. He will need to get 1.5 times the number of his usual calories for the next while. Tips that can help with that are:
You will need to test his ketones at home daily, and anything above a trace means a trip to the vet. Tips on how to test ketones are in the first link above.

If you don't know how to give fluids at home yet, see if the vet will show you how and send you home with supplies for giving fluids. In the mean time, add extra water to his food.

It's very important he gets his insulin full dose each shot. Stall and post here if you get a lower preshot than you know what to deal with.

UPDATE
He came back now from all day at the vet
He wanted immediately food
I mix some wet food with spirulina and he ate it
He look very stressed and his back leg look like floppy
Tomorrow early morning I need to bring him back to the vet for another all day of fluid

they just told me today not to give him insulin tomorrow morning as they will take care of it
and that is too soon to know if he is better
But the vet was not there anymore and they didn’t know the nurse how many ketones he had ..
tomorrow morning I will speak with the vet

any tips exactly what to ask the doctor ?
I will ask her :
How many ketones he had?
Antibiotic because there was an infection? which antibiotics? ?
Does he has Urinary tract infections?
Can I give fluids at home ?

Any other questions to ask and check?
 
Ask why he has elevated white blood cell count - they need to figure that out. UTI is one possibility, but there are others.

I don't like the idea of him going without insulin in the AM. Will you be feeding him at home first?

I have the appointment the time I usually give him insulin
And yes they told me to fed him if he is hungry any time he want . Now it’s 3:30 am here I wake up and he was hungry I fed him and cuddling a bit
 
How are things today? Have you managed to test for ketones again?
Don’t wait til he asks for food, offer snacks every couple of hours.

Update

I spoke with the doctor - Noah is another full day at the vet for fluid today ! We test ketones with a blood machina and is just showing ketones are present but not how many.

Apparently was very hard to deal with him yesterday!
So they didn’t manage to get his urine and they don’t know how many ketones he has !!
They just had a blood machine like the one of the glucose that show ketones when ketones are present in the blood .

Because they couldn’t get his urine they didn’t manage to check if he has an urine infection but they are giving him a full spectrum antibiotic that I have to keep giving him until Monday

She told me that if he came back home after today eating and with low glucose the Dka episode should be over

But they gonna give me fluid to give him home and if he doesn’t eat again I have to give it to him every 2 hours @Bron and Sheba (GA)

Also the doctor told me that his liver and pancreas from the blood test are completely fine and that she saw much wrost and she doesn’t see Noah being in a life treating situation

Praying for your beautiful boy :bighug::bighug::bighug::bighug::bighug: My boy was doing so well with his numbers but in the last few days he spikes .its so worrisome. Your boy looks like mine from you pic . Hope all goes well for you and your beautiful boy
 
I have no idea what’s going on my profile picture shows up in yours. And plus I have a double post which I have no idea how that happened either and I don’t know how to erase it.
 
Can you ask for some ondansetron or cerenia for nausea in case he doesn’t eat?
To test for ketones you need a bottle of Ketostix or Dia-Ketostix form a pharmacy and test his urine to see if there are any ketones. This is really important as it tells us how he is going.
Also get them to tell you how much insulin they have been giving so that you know what to give once he’s at home.
don’t miss any doses and it you are u sure what to do post and ask for help . Someone is always here.
 
The blood meter that tests for ketones is an option some people use. If you could find out the number than they saw, that would be helpful. There will be some low level of ketones a lot of times, it's above a certain number it becomes concerning.

Fluids every two hours is a lot!
 
Not sure if the brand Keto Diastix is available in Malta.
No matter what the brand, the side of the bottle should look something like this:

The bottom line is the ketone chart. Ignore the top line, the strips do not measure glucose accurately in cats.

Hi everyone
Thanks all for your reply!!! Noah came back home yesterday
I’m very trigger from the vet when I went to pick him up his paw were he had the needle inside was very swallen as the bandage was tooo tight !!!
Plus I asked them how do they know that he had ketones and they show me their GLUCOMETER that I went to check on internet and it says that is show the word ketone as a warning to actually check ketone not that he has !!!!!

I came back home and didn’t manage to collect his urine but I stick the ketone strip directly in the sand after 1 second he pee and ketone seems absent (light pink) but there was sand around the strip, I have hope to work anyway as the glucose square came dark as his glucose was high


I have fluid with me now and his antibiotic
Regarding his health he ate and drank a lot yesterday and he seem now he is calming down finally home !!! Ginger crossed for today! I will try to test again for ketone when he pee !
