Nigel update...need help

I am in quite a state right now with my cat, Nigel who has heart disease, remission diabetes and is now in kidney failure. He was seen by the Vet on Monday and admitted for IV fluids with a BUN of greater than 130 and a CREA of 9.9. After two days of fluids, he was sent home on subq's--100cc's every other day with a BUN of 120 and CREA at 6.6 . He ate three meals yesterday of his canned food and was given maybe 50 cc's of fluids and seemed to be doing a bit better but today has been not good at all. He is refusing food, goes to the water bowl to drink but only drools which I feel is nausea. I offered him baby food but that was refused. At 11:00 a.m. today I gave him 50cc's of fluids and in the afternoon I went ahead and gave him not quite 1ml of metoclopramide for the nausea which is all I could think of at the time. I also have Cerenia here but was unsure if I should use it. My Vet is out of the office today until tomorrow. I'm not sure if Nigel will make it until then. He is just laying around drooling, lethargic and there is a horrid odor to his breath. I tried to join the CRF group but have been having trouble with the "join this group" page. It won't connect me just sends an error message. I am desperately trying to learn something about these BUN/CREA numbers and his bloodwork from Monday and please, will Nigel even survive this??? What can I do to make him comfortable through the night so that I can get him to his Vet in the morning? I don't know what to do....

He doesn't sound good. The foul odor is likely uremia - a build up of toxins in the body that the kidneys are supposed to excrete. He may feel a bit better if you "mom cat" him - take a warm, damp washcloth and groom him a bit. The massage action feels good, and even wiping his mouth out some may encourage him to eat (if his mouth tastes like it smells, I can understand not wanting to eat) Can you syringe feed him at all?

If you press a finger on his gums, then remove it, does the color come back quickly or slowly? If slowly, he may need more fluids. A more crude measure is to pull up a tent of fur, let go, and see how fast it snaps back. If the skin is clearly tenting, he definitely is dehydrated and needs fluids.

I'd be inclined to take him back to the vet ASAP, even an emergency vet. Unfortunately, with everything that is going on, it may be time to let him cross the bridge.
 
lorraine, also type in your BUN and creat levels with BW so easier to see all in one place

- these 2 may be low because the kidney stimulates red blood cell production with erythropoetin

MCHC----39.5g/dL---normal 29.0-37.5----HIGH = Mean corpuscular hemoglobin concentration

PLT---742K/uL---normal 175-600-----HIGH = ? guessing pancreatic lipase test ?
GLU---197mg/dL---normal 71-159----HIGH = glucose

- all these may be high partly because of dehydration
- may be testing for pancreatitis/digestive problems and elevations would support those

If he has pancreatitis, other here have posted that it is painful and that the cat may need pain meds.
 
I don't have anything to suggest since I have absolutely no experience in this matter, Lorraine, but I am thinking about you and Nigel and wish you both the best. I know you are so scared and upset right now, I just went through a bout of something with two of my kitties, they were very sick and I had to syringe feed both of them (the both got sick at the same time), so I know the feeling of helplessness you are going through. Not sure what else I can do for you, but if you need something, give me a shout.

just spoke to Lorraine. her electricity went out.
said nigel has eaten 2 TBS of food and is laying in some catnip right now.
she has a vet appointment at 8AM
prayers all go well and Nigel has a chance here
 
poor Nigel. It really sounds like urea is building up in his system. If you are able to get back online - try to give him more fluids and syringe feed him as much as possible. I hope and pray he stays with you through the evening cat_pet_icon
