Nicky - Advice Needed

Hi Guys,

I would appreciate if you would check Nicky's SS. A couple of weeks ago he was doing so well. I thought he was on his way to be regulated and then his numbers just went crazy. I have been trying not to overcompensate by raising his dose to much but nothing seems to be working. He is just so inconsistent. I decided to start him on a lower carb food. I have just started giving him FF classic salmon and EVO 95% chicken and Turkey which are 1% C. I was giving him FF Turkey and giblets which was 4%. I also just got a new bottle of ProZinc and will be starting that tonight. My plan is to dose him at his usual numbers and if he is still high increase from 1.6u to maybe 2U. I will be able to monitor him closely tonight and the weekend.

The ironic thing is that now that his glucose numbers are higher he is more active than ever. Any advice is appreciated.

I'm not sure Luann. I see he was doing pretty well until yesterday when those number skyrocketed! I'm wondering if that was possibly a bounce...with such high preshots for both, and seeing how low he's going previously...it makes me wonder. There's really no way to be sure though.

I'm hesitant to advise much without knowing what his preshot will be...it's tough because Nicky seems to drop like a stone sometimes! Any way you can get a nadir today? I understand if you can't (work I know) but thought I'd ask...if we can see for sure if today he bounces, it might help determine what to do next.

Thanks Rachel, one of the problems is his nadir seems to change everyday, anywhere from 3 to 5 hours. I will go into work late today and try to get a number around +3 and +4. Fortunately, I will be able to monitor more closely tonight and this weekend.

Hi Luann - regarding the changing times for the nadir - I am no expert at all, but I wonder if it could be due to differing rates of insulin absorption - do you shoot him in different places all the time? If so, maybe you could track that in the spreadsheet and see if there is a pattern - may be way off base but just a thought

Wish I could figure out a nadir on Bubba. It changes daily! Nice drop into the yellows for Nicky.

It looks like it could be a bounce or you need a smidge more insulin. How about getting a curve tomorrow and then decide? If he isn't dropping low and bouncing back up, but is in the yellows and then gets up to a red for pre shot, I'd up the dose. (Except if you are starting a new vial, then I'd wait a cycle or two just in case the new vial is a little more potent). If there is a low green hiding in there somewhere, I'd drop it a smidge.

Actually, I do change the location of the shot. I was also thinking some of the problem was my technique. Getting the insulin in the fat layer verses under the skin. I like your idea of tracking the shot location. Thanks!!!!!

Hi Sue,

I had to leave and go to work. I was able to test up to +4. I don't think he hit his nadir yet and already had about a 64% drop. If he goes back up in the 400-500 would that be considered a bounce? If so, shouldn't I reduce?

I need to read up on how prozinc works again. Is the prozinc all used up at the nadir and that is why the glucose goes up because there is no more insulin?

I too had a ton of trouble injecting and judging by Murphy's sometimes painful reaction to the shot, think I did intradermal maybe even intramuscular injections more than once (you want to shoot subcutaneously). Then my friend showed me to first feel for the shoulders, lift up the skin right there and shoot into the tent I dont' seem to have any more problems

Thanks for this Carol! Sometimes, Bubba acts like I am hurting him also. I need to double check that I am not doing a intradermal or intramuscular injection when that happens and really make sure I shoot into the tent I couldn't understand why sometimes he reacts and other times not. Wondering if those times it hurt him and I didn't shot subcutaneously, if that was the difference of a higher number?
