Newly diagnosed - reaction to increased dosage

Hi all, my cat Conrad was diagnosed in late February. In retrospect, he had been showing signs for awhile (increased thirst) but we didn't realize something was wrong until he declined really quickly - a day of really intense thirst, vomiting, weak hind legs. He also has still-unidentified allergies for which he had previously taken daily steroids under the care of a dermatologist; we do wonder if that might have been part of the picture.

He's about seven years old, usually hovers between 14-15 pounds (and could stand to lose a little eventually). He started on 1.5 units of Lantus morning and night and has had a very positive response in terms of how he acts and seems to feel. He is still on dry food; a prescription hydrolyzed food for allergies (we did a food challenge test in early 2020; nothing seemed to bother him; we switched back to his regular food; increased allergies so we're back on hydrolyzed and in kind of a holding pattern there). I know wet would be better but I'm worried about allergic reactions and the switch because he just hates wet food.

The vet did a curve on Thursday and his lowest point was still in the low 400s, so she suggested increasing his dose to 2.5 units morning and night, which he has now had three times (Fri PM, Sat AM and PM). Last night he vomited while we were sleeping and then barely ate this morning. I didn't give a dose Sunday AM because that felt like a lot of variables and we aren't testing his levels ourselves at this point - I worried he had barely anything in his system and that maybe he was reacting to the higher dose. He's still showing increased thirst, but seems otherwise fine - playful, wanting attention, etc - nothing like when he declined just prior to diagnosis.

So I guess this is an introduction as well as asking for some advice on if the vomiting could be related to the increased dose / how to respond if it happens again. This site has been so helpful; thank you all for the information you share!
 
Hi Amy, I'm sorry you haven't had a response yet. I don't know the answer to your question but let me try tagging @tiffmaxee to see if she can help you. I believe she's a Lantus person. If you don't get a response before long you might try posting on the Lantus board. There are a lot more eyes there.

Welcome. That’s way too big an increase. Are you home testing? Do you have a pet or human meter?
 
If you don’t have a human meter and are in the US you can get Relion at Walmart. You can also pick one up at a pharmacy.
 
We're not home testing at this point; I feel like we're still getting the hang of the injections in the first place. I will start reading some of the info about it here to get a sense of what we could be doing; I do feel like it would be nice to have a better sense of what's going on with him. I'm hoping he'll have more of an appetite for dinner to give him some kind of dose; I'm wondering if 2 units might be what I go for tonight and check in again with the vet in the morning.
 
Increasing by that much when not testing at home can lead to a symptomatic hypo. When Max was first diagnosed I didn’t test but saw my vet around mid cycle every few days. We don’t know if he was already having symptoms. I would not shoot 2.5.
