Newly diagnosed need advice/have questions

Hi all- hope you can give some advice. Sorry this is long.

My cat Punch is 13 yo male I've had since 8 wks old. Punch has only ever eaten dry food- although he has always been a chow hound and will eat anything and often steals our food when left unattended. 5 yrs ago he got a bad bladder infection and after that I only had him eating wellness dry or solid gold. We got a second cat this year back in march. With the added cat the food bill got pricey and the new cat was having lose poop so about 3 months ago I switched both of them to Max cat dry and both seemed to be doing fine. About 2 months ago we noticed Punch was drinking a lot of water. the water bowl is next to our bedroom and we would hear him at night visiting frequently. He also lost his hearing for the most part. I took him to the vet last week thinking kidney issues and they ran a full blood work and urinalysis and everything came back great but glucose was 300. No ketones in the urine though. They ran the other test for glucose (cant remember what its called but more definitive) and the vet said it was elevated but not sky high. She told me some cats can be managed with diet, some need insulin short term and then can be managed with diet- she wanted to put him on insulin. I came to this site and others an read up. The night I got the high glucose resutls I immediately switch to all canned no grain canned food. That night no frequent visits to the water bowl. I told the vet and she agreed I should try just diet for a few weeks and see how it goes.

Since friday night, he is only eating wellness grain free and some trader joes tuna for cats. I would say his drinking is less than 50% of what it was from what we can tell. I got a glucose reader today and took a reading 45 minutes after he ate a full bowl of canned food and it was 101. Im new to the glucose reading but I work in a medical related field and have a lot of experience working with research mice so Im not new to animal handling and blood taking. It took some work to get the blood but he's a pretty easy going cat.

He was never acting like he had anything wrong (not other symptoms)- I just knew that kind of drinking is not a good sign. He acts like he has died and gone to heaven with all this canned food... it suits him just fine. I feed him twice a day. I also got Wellnes Core dry hoping he can eventually eat some for when we are not around at feeding time and to feed the other cat just in case Punch manages to steal a few bites. Is wellness Core a reasonable option?

Of course I would like to avoid putting him on insulin in possible. What do you all think- was it just a bad diet for him on Maxcat and does it sound like diet management may work for him? Is it a good idea to try it or shoudl I not wait to put on insulin? How would you proceed from here? i really trust my vet- she has been right on with health issues with my pets in the past and Ive been going to her for 10 yrs.

You have done a great job so far. Yes, the wet lo carb is the way to go. And since you are dealing with a cat that is barely diabetic (110 is not a bad number - you normally would not give insulin under 200) if you can go to all wet, that may do it. Some kitties are carb sensitive and even a grain free dry food can raise their blood glucose levels.

For a kitty off insulin and in remission, you are looking for numbers in the 40-70 range most of the day. Feeding will usually lower the blood glucose levels if the pancreas is working.

If I were you, I would start testing regularly, in the morning and evening and after he eats. Keep track of your numbers. You might try to feed the lowest carb foods to see if that makes a difference in the numbers. Try 3-5% carbs, but avoid fish more than once or twice a week. (Too much mercury and the chance of his eating it to the exclusion of everything else.)

You are way on top of the learning curve....good job.

Personally I wouldn't feed either cat dry food...I have 11 (only 1 is a diabetic) and they all eat exactly the same thing as my diabetic, good old fashioned Friskies or 9-lives canned that is less than 8% carbs. Max, my sugar cat didn't have a great start in life and most of history is unknown to me. What I do know is that with a diet change and a small amount of insulin he was OTJ (off the juice) or in remission in just a couple of short weeks. He was 485 when dxed so with Punch as borderline as he is I would just switch them both to low carb canned and see where you go from there.

My 10 civies (non-diabetics) are even better off for the switch, their coats have improved and so has their over all health.

Thanks- so is it likely that the reading I got with the glucose tester is fairly accurate? Its the true2go from CVS (good friend works at CVS so got me a deal). I will keep testing. What about the timing of the testing. I thought right after a meal would show highest but not sure 45 minutes was a good time?

He's hiding from us now.. guess I am going to need to give him treats after poking his ear in the future. He is very food motivated.

Also, he is overweight so hoping to get his weight down and that may help some too.
 
Treats help a lot!!!! You might want to test before he eats and then about 45 minutes to an hour after eating to see if the food drops his BGs. He should be highest before eating, if his pancreas is working even somewhat, food should bring him down.

Just makes sure his treats are low carb as well. I use either bits of raw chicken or Wal-mart sells chicken jerky for dogs that everyone in our household loves. They are pure chicken and I just take scissors and cut them into little pieces for the kitties. They don't effect Max's bgs at all and he will do anything for them. They are sold under the brand name Waggin Trail and are over with the dog treats.

Ok thanks for the info- for some reason I thought the BGs would be highest after eating. I will try to get him tested prior to eating tomorrow evening and compare it. Thanks for the help!
 
If he is a new diabetic, unregulated, then the numbers after eating will be higher. But if his pancreas is working a little (or a lot) the number will go down.
 
Impressive. Sounds like you might be able to avoid the insulin. Crossing fingers for you. I am sure your meter is pretty accurate, based on what I've read here over the months, many people use many meters that they're happy with. Any human meter should be just fine. Continued good luck...
