Newcomer with feeding issues

Greetings everyone,

Let me give you the rundown:

My kitty: Her name is Gorda (spanish for fatty), she's approximately 11 years old, and has never had any health issues before. She's operated on a free-feeding schedule with dry food, and has always maintained a "festively plump" physique.

Two weeks ago, I took her to the vet, because she was losing weight precipitously and drinking a ton of water. I was worried her kidneys were failing, but they weren't worried about it. I basically had to talk them into doing a blood test, at which point they found her blood glucose was close to 600 mg/dL. So, she's diabetic. They gave me some syringes and some Humilin - N, and told me to inject her with 2 units twice a day. I asked about diet, and they said that I could keep free-feeding her. No mention of low carbs.

So, I come home, do some online research, and see that everyone in the world suggests a low carb diet. My cat has never had a problem with canned food before, so I figured I'd give it a shot, along with continuing the insulin shots. The first day, she ate voraciously, and seemed to love the canned food. She stopped drinking so much water and her litterbox situation was rosier. But then, the next day, she just... stopped eating. I was very concerned about this, so I did my best to feed her using a water + wet food + wide-mouthed syringe to feed her. But I could never get her to eat enough, and her weight dropped noticeably in three days. I went back to dry food, but she wouldn't eat that either.

So, I stopped giving her insulin shots, and learned a new method of feeding her - smearing food all over her so that she has to clean it up. it's not pretty, but it worked. Again, the first night she seemed almost back to normal and even went back to eat on her own, but by yesterday she was in rough shape again. So I continued to smear-feed her and started giving her insulin again last night, at 2
units/time.

More importantly, I finally bought a home glucose monitor this afternoon. I'm doing the tail-end of a curve, and she was at 337 mg/dL before her shot tonight. But I really had to pester her into eating again before the shot. She won't touch anything on her own. So I've combined lunchmeat with some food-smearing.

So, I guess my question is this: What am I doing wrong? I know Humulin-N is not the best, but it was $61, I'm poor, and she's shown no signs of hypoglycemia after the shots. I feel like the drastic changes to her diet have thrown her for a loop, but now she won't eat dry food either. And I can't force her to eat that. I'm sick of making her eat when she doesn't want to, and I'm sick of watching her waste away.

I plan on running a glucose curve each day for the next three, and then seeing if I need to adjust the dosage after that. Is that a good plan? Any advice on what I can do about the food?

She must eat. There are other reasons she might lose her appetite like pancreatitis, which a vet would have to test for. She could be in pain and that would affect her eating. You could try syringe feeding: Syringe feeding or some of these ideas: Assisted feeding But if it is something like pancreatitis, you need help from the vet. I know money is tight, but a diabetic should not be without food. It can cause worse, more expensive problems.

I think a curve is a great idea. It will help you see how the Humulin is working and whether that is part of the issue. You are right - Humulin is not popular here. But cats have been regulated on it, with a lot of testing done by their owners. But, for me, the eating is vital.
 
Have you tested for ketones? You can get test strips at the pharmacy by the diabetes supplies. Ketones can make a cat feel poorly and not want to eat, but if the ketones are high, it can also be life threatening. As far as eating, you can try chicken, meat baby food with no onions (we use Beechnut), tuna, anything you can think of that will get kitty to eat. Try warming the food slightly to make it smellier too. Keep trying, as they must eat.
 
First - I've made her eat - but probably not enough - maybe half of what she actually needs.. I will use the assisted feeding method in the Youtube video if I have to, and make sure she's getting enough calories twice a day.

As for ketones, doesn't her blood sugar have to be really high for that? I've only tested her twice so far, and she came up at 280 mg/dL and 337 mg/dL (at which point I gave her the insulin). And I haven't noticed anything strange about her breath.
 
Ketones have just as much to do with infection and not eating as high numbers. Some cats have ketones while their numbers are in the 200s. You used the ketostix and checked her urine, right? Was it in the normal range?
 
I have not tested for ketones but will tomorrow.

For future reference, please don't offer advice in the form of "You've done this, RIGHT?" It's condescending and makes a tough and guilt-ridden situation even tougher and more guilt-ridden. You were new at this once too, remember?
 
Sorry, it is hard to communicate in this setting. I didn't mean RIGHT? I meant to double check that we were talking about the same kind of testing.
 
I agree... hard to communicate. I appreciate the help, really. It's something I hadn't done enough research on. She's definitely lethargic and dehydrated, along with the lack of eating, but doesn't show any of the other symptoms. In any case, I'm sufficiently worried about DKA that I'm taking her to the vet first thing tomorrow morning. It really would make sense if that was the problem. And in any case, I feel like she just needs an IV drip at this point to get some fluids in her.
