New vial of lantus- do I need to reduce dose?

In the next couple of days I will be starting a new vial of Lantus.
Do I need to reduce the does that I am giving Tennie to make up for the potential increase in "potency"? He is currently getting 4.25 units bid.

To complicate matters he is due for an increase. We are getting better numbers- but nowhere as good as pre-pancreatitis. Boo!

Any advice would be helpful.

How long since pancreatitis? He he still on any support meds? It can take a long time or short time to recover completely. I have never decreased the dose with a new pen or vial but I watch a little more diligently to make sure Max doesn't drop too fast or too low. I wouldn't increase with the new vial though just in case that is part of the higher numbers but rather give a few doses first to see what happens. I don't give dosing advice though so let's see what others say about an increase.
 
Its been almost 3 months since the pancreatitis. He has not taken any support meds for about 2.5 months. He is eating and moving around well, but I am still keeping an eye on his eating. So far so good.
I will probably hold off on the increase for a few days once I start the now vial. But I was interested in what others do.

We don't usually reduce the dose when starting new insulin. However, I wouldn't do an increase and start a new vial at the same cycle. He's not seeing any greens at this dose. I think you could increase now and the new vial in a couple of days.
 
No, we've never seen a green. Before the pancreatitis he was getting close but the vet said not to shoot under 200.
But that was before I found you wonderful people. Now I am just getting prepared for the day it happens.

I don't think I have enough to go more than 1 or 2 cycles with the old vial.
I may have to start the new vial than increase after a few days
 
I wouldn't wait a few days after changing. If the old vial has lost potency you will see the change in the first couple of cycles. IMHO you have been holding the doses too long - particularly at 3.5. This means that kitty has more time to recognise those nasty numbers as normal and makes it harder to bring them down. Just give the new vial 2 or 3 cycles before increasing.
 
Yes, that sounds like a good plan. It looks like I can do 1-2 more cycles before switching to the new vial. Then I dont plan on waiting too long before increasing, just the 2-3 cycles like you suggested.
I realize that I may be holding doses too long. I am just paranoid about him going too low and need to get over it if I ever want him to at least get regulated.
I fell like I am getting close to a good dose for him though. Hopefully we will get there soon.
