? New to Lantus, asking for dosing advice 1/1/2026 AMPS 243 +5 254

Need your advice about what to do in the next few days, and also help understand the protocols.

Today is the second day of Lantus, dosage 0.25 units twice a day. Previously, it was Levemir. So far I see an increase in glucose, but I hope it is temporary. If I understand correctly, then for the first week I have to keep this dosage and only after 5-7 days can I increase the dosage to 0.5 units twice a day? Feeding exclusively wet canned food.

A separate question about vitamin B12 to help a cat with diabetic neuropathy. There is a slight weakness in the paws, he can no longer jump onto the windowsill, he needs steps. Although he is only 8 years old and weighs 5 kg. We live in Ukraine, if this is important for choosing a drug. What drugs and how to give?

Of the chronic diseases, he has chronic pancreatitis (now it does not bother him), but the herpes virus infection cannot be overcome. I have been giving FAMCICLOVIRUM twice a day for two months, 250 mg, and conjunctivitis regularly worsens, which also requires antibiotics, without them everything is useless.

I take good care of him, but I feel that I am not doing enough and I want to help him regain his desire to live and enjoy cat activities.

See your thread in Health for a European recommendation of methyl b12, How to assess the chances of remission? The hind leg weakness is called neuropathy, not encephalopathy which is a brain disorder.

You can't ever get rid of the herpes, just manage the symptoms as they occur. Have you tried Cidofovir? It's a antiviral.

Since my cat was diagnosed with diabetes, the herpes symptoms have not gone away or have only gone away for a short time (1-2 weeks). Cidofovir is not sold in my country.
Tearing after eating has been with us for the last 7 months
 
Hi Alisia, about Asik's diabetic neuropathy, the advice you were given in your other thread about Vitamin B12 is great, and that Asik needs the methylcobalamin form specifically.

I had a hard time finding this in a pet medication for my cat. Everything had B12 in its cycobalamin form. However, I take B12 methyl tablets, so I divided mine into the same dosage Lu would need, and give it to her that way. So tablets or capsules sold for humans might be the answer for you.

I see that this is Asik's 2nd day on Lantus, so tomorrow or after a few days you could ask again for dosing advice if you don't get it today, as this is a holiday for many.

The recommendation to stay with the starting dose for 5-7 is only if the cat is new to insulin. Asik is not new and switched over from another depot style insulin. That means a couple things. First, the Levemir depot is depleting and the Lantus one is building. It's hard to see what this dose is doing right away in this transition time. Especially without PM tests, which you didn't get last night but I'm glad to see you got tonight. Next, you don't need to stay with this dose for 5-7 days, you can evaluate it after 3 days, providing you continue to get the number of tests you need to follow Tight Regulation for dosing.

My vet recommended this Idrop product with hyaluronic acid for my kitty that gets herpes flairs, especially if her eyes are not infected and oozing green junk. In which case an antibiotic drop is needed. Turns out I use the same Idrop product in my own eyes and the vet confirmed they are the same ingredients but the human version is cheaper.
