? New on Prozinc + low eye pressure, on prednisone acetate drops

I wasn’t really sure where to post this. This is a question for a good friend of mine.

Her cat, Stewart, was recently diagnosed with diabetes. His vet put him on Prozinc. However, he also has multiple other health issues going on, so I figured the health forum might make the most sense. I have been trying to help Pam – but I know very little about Prozinc (my boys are on Lantus) and I am not sure about the eye issues, etc.

So, here's the info:

Stewie is a 15-year-old long-haired cat who has always been pretty healthy.

August 16, 2016: Stewart went in for an exam and was diagnosed with low eye pressure and uveitis, especially in the right eye. He also has a hole in one of his molars that needs dental surgery. BP test given, senior profile blood work done. Mild increase in one liver value, normal CBC. Glucose level only slightly elevated (at the time, they attributed it to stress – he HATES going to the vet). Cause of uveitis was unknown. Dr. E prescribed prednisone acetate drops 3x a day for his eyes.

Aug 18, recheck of eye. Still low pressures, prednisone acetate drops continue.

Aug 23: started oral prednisolone, 5mg, 1 x day.

Aug 30: right eye was worse, pressures were 8/9/10/9 on the 4 times she tested it. Prednisone acetate eye drops continue.

Sept 6: Started Clavamox for dental.

Week of September 12 water intake increased.

Week of September 19 water intake tripled.

September 23rd, BG tested prior to dental surgery and was deemed too high for surgery. Stewart was kept for a BG curve. I unit of insulin (Prozinc) given around 8:45 am.

BG 8 am 378 (fasting)

BG 11 am 427

BG 2 pm 344

BG 4:30 pm 276

BG 9 pm (at home) 99

Prescribed 1 unit of Prozinc insulin 2x day, new diet (Evo). Stewie won’t eat canned food. Oral Prednisolone was discontinued. Prednisolone acetate drops continue.

Sept 23 to Sept 28: 1 unit insulin (numbers are mostly in the 300s, spiked to 521, not any lower than mid-200s)

Sept 29 to Sept 30: 1 1/4 unit insulin (Mostly 400s, a couple of high 200s, some 300s and spiked to 579 one time).

Sept 30 to Oct 7: 1 1/2 units of insulin (Average of 350. Two high spikes of 522 and 544. Some high 200s)

Oct 8 to Oct 14: 2 units of insulin (Spiking in to the 500s more than 5 times. Averages are in the mid-400s. Lowest was 288 on the day the dose was increased.)

Oct 15 to current 2 1/2 units of insulin


Sorry, Stewie doesn’t have a spreadsheet set up. My friend works crazy hours, so there are not a ton of mid-cycle tests, but she always has the AMPS/PMPS and at least one mid-cycle test (anywhere between +3 and +7). She has been increasing him slowly on days where she is home to monitor.

For the folks who are more familiar with Prozinc and/or eye issues: What would your advice be? Keep increasing the dose? Consider switching Insulin? The vet wants to continue him on the prednisolone eye drops for now and she is not comfortable doing a dental before he is more regulated. But is that even realistic (getting him regulated while he has all of this going on)?

Any advice would be appreciated. I feel way out of my comfort zone on this one...
 
Without BG before shot, and then periodically like every two hours between shots, I can't give advice on dosing. The dose is based on not going below about 80 (USA) between shots. An average BG over a day is really not useful. The highest BG is usually before the shot but van be 1 to 2 hours after shot
Oral Prednisolone will raise BG. Prednisone acetate eye drops will likely raise BG since steroid containing ear drops raise BG.
 
Are you sure it is low pressure? Uveitis can cause high pressures in the eye in which case treating with prednisolone makes more sense, it can also be associated with auto immune conditions.

From what she posted:

"Aug 30: right eye was worse, pressures were 8/9/10/9 on the 4 times she tested it. Prednisone acetate eye drops continue." it does look like low eye pressure


According to Merck vet manual normal is:

I see Stewie is 15. I'm not sure whether this may be helpful to your friend but Saoirse's opthalmologist told me that eye pressure may be in the lower part of the reference range for older cats (advised me that normal intraocular pressure for a 16-year-old cat was 10-12).

I will try to get her to upload the numbers into a spreadsheet. I saw them on paper earlier today and there is nothing even remotely close to 80. Even the lowest numbers are still above 250.

It might be worth doing diagnostic tests for pancreatitis; it is one possible issue that may lead to uveitis.
Thank you! Very interesting... pancreatitis was actually brought up at some point, especially since he wasn't eating well, is not not grooming himself and is just not cting like himself. I don't think they ever pursued that though.
