Hi everyone! My name is Maria and my cat’s name is Karamela! We received the official diagnosis 3 weeks ago! She is 8,5 yo and 4 kg! On July of 2023 she had pancreatitis, that affected her kidneys but after treatment and renal food it was restored! We discovered diabetes in the following up exams! No symptoms! We are not on insulin yet! The doctor proposed caninsulin but I want to propose Lantus. I am confused about the dosis and I need your opinion! This week we will definitely start insulin. Thank you :)

You have done your homework! Glargine (Lantus) is a much better insulin for treating feline diabetes than Caninsulin. If your vet is skeptical, the guideline for treating diabetes from the American Animal Hospital Association supports using Lantus. They haven't recommended Caninsulin since 2018.

Is Karamela still eating renal food? There's a very good chance that the food is higher in carbohydrates than is recommended for diabetes. The challenge is how to treat both conditions if the kidney problems are still current. In fact, it's entirely possible that having switched your cat to a kidney diet is one of the things that contributed to her high blood glucose numbers. Since you've not yet started insulin, I would suggest making sure that you are feeding Karamela a low carbohydrate diet. We consider low carb as less than 10% carbohydrate -- although most members feed their cat in the 5% range. If you switch her diet, testing her blood glucose will tell you if the food change is all you need to do or if you still need to start insulin.
 
Thank you for your response I thought the same she used to be in renal food! Her kidneys right now are perfectly controlled. After the diagnosis I was told to change to royal canin diabetic. After research I saw that it wasn’t appropriate! So now I changed again to an approved low carb wet food. Purina pro plan dm diabetic. We have just a few days in this low carb food do you suggest wait more until insulin? Or start the next few days? Thank you

Hi Maria! I thought the low carb food suggested by your vet and Karamela had been eating until yesterday contained peas in it.You switched to a low carb food ( Purina DM mousse) yesterday. So she's been eating low carb food just one day. Correct me if I'm wrong.
 
Since Friday evening she is in low carb. Before that she was in royal canin for a week and then brit diabetic for a very short period like a few days ( that indeed contained peas). and since Friday evening she’s in purina.

Lantus is a great insulin to use so hopefully your vet will be on board with using it. There are published studies in well known veterinary journals that conclude that glargine works well for cats.

Have you been testing since you switched your cat over to a lower carb diet? I would estimate you would see a change in numbers in 5 - 7 days.

Also, many cats end up not liking the Purina DM. The ingredients are not wonderful.. I would look over the lists that people posted for you and see if there are foods that your cat prefers. I'd also try to find varieties that are low in phosphorus.
