new member (5/2/24)--inconsistent insulin levels

Hi! I hope I'm using this correctly; please bear with me if not!

My five year old male, Bandit, was diagnosed with feline diabetes back in March with numbers in the low-mid 300s as a result of the medication he'd been prescribed to treat his feline asthma. We've been treating him with one unit of insulin glargine twice a day, and his numbers have been getting steadily better. We've also managed to get him fully off of his asthma medication, and he's made quite a bit of visible improvement.

We went to do a partial curve Wednesday evening, and his first reading was 142, which was lower than it's ever been. We thought our glucometer was acting up, so we tested it with the control solution, but it was fine, so we did another reading to make sure we hadn't messed up somehow. We got back 174, which we were happy with, since he was pretty stressed from having us do a second reading. Five hours later, he was at 116. Our vet recommended not giving him insulin if he's under 225, so we skipped his dose the next morning and checked his levels that evening, and he was at 200, so we skipped his evening dose as well. When we checked him this morning, however, he was back up to 316.

We're waiting to hear back from the vet, but in the meantime, has anyone experienced something like this where their kitty has a major dip (without getting dangerously low) and then climbs back up? We thought we were just getting lucky and he might be going into remission, but with him back at 316, that seems unlikely.

Hello and welcome. Good to see you are home testing Bandit.

What sounds like happened is Bandit responded to the insulin and his numbers got to some nice levels. Then the insulin effect wore off and he went back to his previous blood sugar levels.

What food is Bandit getting? It's possible that a food change might also help nudge him towards remission. I presume the asthma medication was a steroid?
 
He’s been getting Fancy Feast Grilled twice a day! We made sure to follow our vet’s recommendation of >40% protein and <18% carbs. And totally right about the asthma medication.

We tested him last week, and his numbers weren’t as low when we did, which is why we were shocked to see something in the 100s. He’d still been in the 200s with his insulin so far. Maybe he just flipped a switched and started responding to the insulin even better, I guess?
 
The Fancy Feast Grilled are all a little too high to feed as a daily diet. We suggest feeding foods in the 10% or less carbs range, most people feed in the 3-6% range. This food chart lists a lot of the foods, including FF ones. Make sure you are testing regularly before switching foods, as his insulin needs could drop some more.

Dropping the steroid can also make a difference in his blood sugars.

Thanks for the chart! That's actually the one we used to pick out his current food based on the information we were given by our vet. She sent it over to us along with those numbers. I'll take a look at some with a lower carb percentage.

And that's been the goal with dropping the steroid. He'd been on a pretty high dose before, and we've been gradually lowering it. The difference from last week to this week has been very minimal, but it may have just started to really leave his system, I suppose. I know steroids tend to hang around for a bit.

Sorry I don't have more BG data to give. I saw the information about the spreadsheets, but we've only been testing him sporadically (at our vet's instruction) as it corresponds with dropping his steroid dosage, so we don't have a huge amount of data, nor have we been keeping a spreadsheet. I plan to keep one from here on out, but I don't have the data for one at the moment.
 
Also meant to add that we'll take another look at the chart to find something with an even lower carb percentage!
