New kitty diagnosed - starting Lantus

Hi, everyone. It's been a while. I hope everyone is doing well and the kitties are feisty (in a good way).

I'm asking for a friend...Her cat was just diagnosed with diabetes and I asked what dosage of Lantus her vet had started the cat on. She told me it was 0.3 ml 2 x a day. Would that be the same as 3 units 2 x a day (in which case, if memory serves, that's somewhat of a high starting dose)?

Yes, that would be considered a high starting dose.
The usual generic recommendation is 1u BID.
I have see two different protocols lately.
The TR protocol recommends this -
.25u per kg of body weight per day BID. So it depends on what her cat weighs.

The other protocol, which isn't the same, recommended .5u per day BID per kg of body weight if the cat's BGs were above 360, but if the BG was less than that, then it was .25 per kg BID like our TR calls for.

So do you know what kitty weighs? And do you know if there is some logical reason for the 3u starting dose, like maybe another condition or two on top of just diabetes? I'm almost certain that unless there is something else going on with kitty, then the recommendation here would be to start at 1u and stick with that dose for several days while collecting data.

Can you contact her and make sure that is exactly what the vet said? Because a u100 syringe has a capacity (I believe) of 3/10cc (which is the same as 3/10ml). SO a dose of .3ml would be a full syringe, which makes absolutely no sense, because that is 30 units!

Can you see if the vet said "3u" specifically? Or ".03 ml"?

P.S. - I only used u40 insulin and syringes, so I have never seen a u100 except as a picture here. Can someone who uses u100 syringes chime in?
 
Yeah... sounds like your vet meant 0.03 ml. That would be 3u in a u100 syringe. Which is still a pretty high dose to start at. 3u of a u100 insulin (Lantus) in a u40 syringe would be 7.5u, which is yikes! Make sure you're using u100 syringes or do the proper math to convert it to u40. 1u of a u40 insulin is equal to 2.5u of a u100 insulin.
