Need Help with dosing

I need some advise with my cats dosing. We just started home testing over the weekend. Seems she has been running 270s to 300s. I saw a 105 yesterday morning (maybe an error as she was 279 this AM) so we didn't give her any insulin. Here is her current spread sheet. I'm having my husband give her 2 units this AM. Does that seem appropriate or should we increase? She recently changed to wet food this weekend and does the friskies pates from Wellness core dry.

Right now it is hard to answer that question as there is very limited data to base a decision on...With Lantus you don't base dose raises and decreases on the preshot values, those are more to make sure they are high enough to give insulin too in the first place. With Lantus you want to look at the nadir or the lowest point the insulin takes them. So if you can try to get a few +6 (6 hours after injection) that will tell us more on whether she needs to go up or down in dose.

Now for a couple of questions for you...I know you are new to home testing but how long has she been on 2u twice a day? And how did you get to 2u? With most newly dxed cats we recommend starting at no more than 1u twice a day and some even start at .5u twice a day and then slowly raise as their numbers indicate. If she hasn't been getting insulin for very long and with the recent diet shift I would be inclined to say drop back to 1u twice a day, hold the dose for a week while trying to get not only her preshot values but also some mid-cycle tests to see how far that dose is taking her down. It could be that 2u bid is going to be too much insulin with the removal of the dry food. And I would rather see her run a little higher for a few days until we know for sure she needs more than to drop too low for you.

Too high kills slowly, but too low can kill very quickly..or as the saying around here goes better to be too high for a day than too low for a minute.
