Need Help with Dose

Please take a look at Nonitta's spreadsheet and give me your advice regarding the right dose amount, i've just moved her to Lantus, starting with 0.5 IU.

What was Nonitta's last dose of Vetsulin, and how were her numbers on that dose? Usually when a cat changes to Lantus from another insulin, we will take into consideration what the dose of the old insulin was.

She was on 1.5 IU of Caninsulin, i have her old curves somewhere i will try to find tomorrow, but she never went below 15 after shot, and her pre shot numbers without food are usually around 18.
 
You've found the right place. these folks have a wealth of knowledge and support that quite literally has saved my guy...and everyone is very willing to share.

Did you skip the AM dose, if so,it seems from your SS that there was no reason to do so - with Lantus, when you begin, you need to hold the initial dose for 5 to 7 days (unless the BG numbers tell you otherwise, which yours, in your SS don't seem to...?)

It might be best to keep on with your starting dose and not skip a dose unless you have good reason to do so... if you have skipped a dose now then no harm no foul, you can resume at +12 and carry on from there...

Anyhow I'm sure wiser eyes than mine are on you and will correct me if need be - so hang in there, and in the meantime, welcome to Lantus Land!
 
no didn't skip any dose. i give her a shot every 12 hrs, please click on the world/international at the bottom of the spreadsheet, i'm using that one
 
Oh I see. I was looking at the SS that is linked in your Sig. (Perhaps you need to link to the World SS?) Folks here, of course, are used to USA units,so please excuse me? I'm not sure how people have dealt with this in the past, but I hope this can be resolved without too much trouble...!

if she was on 1.5 units of Caninsulin and staying high, then 0.5 units BID of Lantus is a little bit low for a starting dose.

There are a couple of choices for how to proceed. You could just say "oops, I started too low" and start over again at 1 unit (then hold that for 5 days), or you can stay with 0.5 as your starting dose and just be a bit more aggressive with your increases. By that, I mean to increase after only 6 cycles instead of waiting a full 5-7 days on the first dose, then increasing according to the numbers. The protocol allows for increases to be done sooner in the case of cats with high, flat curves.
 
then i will say OOPS ;P

I will do it to day at the AM shot .. cuz i'm already done with the PM one.
the protocol gives it 3-5 days to increase, and it's already been 3 days .

I think you made a typo for your am dose amount. In the world sheet it shows as 0.05 and in the US sheet as 0
I assume you meant to type .50
