Need dosing advise

My cat is on 2 Units of Lantus 2x day. He is a picky eater (and a grazer) so I have to mix it up. Food: Fancy Feast, Purina One Grain Free, or Purina Pro Plan True Nature ALL Pates with water and at night a little of Dr. Elseys Clean Protein Chicken. He loves kibble so he gets a little to graze on during the night. today I was able to do a curve. Pre shot 8:50 am 308, Shot at 10:15 am, 12:25 am 300, 2:35 pm 242, 4:40 pm 271, 8:45 pm 316. I use the True Metrix Air human device. I always test before shot and he is running right around 300 before both shots. Do I need to increase his dose? He had a dental a couple of months ago and had 8 teeth removed. I was hoping his numbers would get better but it seems it has been harder to get them down since the dental. Wanted to check here before taking him back to the vet. Let me know if you need more information. Thank you
 
We're going to need more information in order to be of any help. I'd encourage you to read the sticky notes at the top of the Board.

How long have you been giving your cat insulin? What was the initial dose and how was the dose changed? Has your kitty had any medical issues besides teeth? Any history of diabetic ketoacidosis?

Because Lantus is a depot-type of insulin, doses are held for a period of time so knowing when the last dose change was is important. Since you are feeding your cat some kibble, you are constrained to using the Start Low Go Slow approach to dosing.

We rely on having test data logged on a spreadsheet. This allows us to see the bigger picture -- not just one cycle's worth of information. This is the link to information on how to set up a spreadsheet. Without having at least several days of test data, we really can't answer your question about a dose change.

Lantus onset (when the insulin starts to work) is about 2 hours after you shoot. This being the case, you do not have to wait after you get your pre-shot test. You want to test, feed, and shoot in rapid succession. Most people here do this in 10 minutes or less.

Just an FYI....the Pre-shot is the number you should get immediately before feeding/shooting. We Test (to make sure they're high enough to give insulin), Feed (to make sure they're at least willing to eat) and Shoot....all within 5-10 minutes.

Since Lantus/Basaglar and Levemir are gentle insulins, the shot doesn't usually start to "kick in" for 2-3 hours (for Lantus/Basaglar), 4-5 hours for Levemir so your cat has several hours to eat before the insulin starts to work.

Thank you for the info. He started out about a year ago in Prozinc. But I switched vets because they were pushing the prescription food and giving me a hard time about it. The new vet switched him to Lantus 2U. After he had been on it for a couple months we had the dental done. He has been on 2U since the dental. The only kibble he gets is Dr Esley. I will have to work on the spreadsheet because all of my numbers are in the app.

Great. We will look forward to seeing the data.
Just checking that you test, feed, then give the insulin all in the same short period of time. As Chris said you don't need to wait for the food to take effect before shooting, becomes Lantus doesn't have an onset for a couple of hours.
