My cat recently started Lantus. His vet wants to increase the dose to 2 units twice a day. I have read about the "Start Low, Go Slow" approach and I am concerned that might be too big an increase. I would appreciate any help and advice that anyone can give. Thanks.

I can't give dosing advice other than to say that a 1unit increase is pretty big all at once, especially since you haven't been using it for very long. I'm sure someone else will be along with dosing advice, but I have seen many times where the suggestion was to go up by .25 to maybe .50 at a time, hold it for several cycles and then reevaluate based on the nadir.

My vet recently wanted us to go up by .75 all at once, but I was uncomfortable with that (confirmed with the experts here) and chose to only go up by .25. We have started to see some lower numbers with just that much of an increase.

If you don't get any advice here (this board is sometimes pretty quiet), you may want to also post on the tight regulation board. It is usually more active.

Good luck,
Kathie

Your instincts are right on target you don't want to increase by full units. At most you want to only increase by a half unit at a time and usually only by .25u.
