I run a small Abyssinian rescue. Currently fostering a diabetic cat. Big believer in home testing. My diabetic foster is due to go to his new owner on 5/14. Same day I nailed down those plans I got a call from someone else who needs to surrender his cats. One of the two was just diagnosed with diabetes.

I cannot take his cats in until my current 2 fosters go the weekend of 5/14. The owner is afraid of needles. He is paying a vet tech to come to his house twice a day to give insulin. No one is testing. They are starting him on 3 units of ProZinc twice a day. Seems like a really high starting dose to me. I'm worried. The owner was going to remove midday dry food and I told him not to do that at this point.

The owner lives on Long Island, Malverne. I live 3 hours from him. I'm wondering if there's anyone in the group who might help this guy for a few weeks. You might even be able to charge him a small amount since he's paying a tech.

Am I wrong to worry about this starting dose? I know I started my current foster out at 1 unit then adjusted based on home testing.

Am I wrong to worry about this starting dose?
I've not used Prozinc but I'd be concerned about a 3IU starting dose of any insulin.

There is an FDMB guide for using Prozinc. From the guide:

Starting Doses
The recommended starting dose for ProZinc or BCP PZI is .5 units or one unit twice daily. It is suggested that if the initial numbers are high, doses can be changed every three cycles, increasing by .5 at a time. If the initial numbers are in the 200-300 range at preshot and midcycle (nadir) numbers are not too low (not 50 and under), increases can be made .25 units at a time. During this early period, especially if wet low carb food is replacing higher carb dry, monitoring is very important. Insulin plus a diet change can result in sudden low numbers or in a gradual lowering of numbers overall, depending on the cat.

You were also right to counsel against any changes to the cat's dietary carb load. Without BG testing any reduction in carbs, especially on a rather high dose, could be very dangerous.

Yes, it's this dose of insulin that has me in a panic. I happened to be at my own vet this afternoon and she was shocked that anyone would start a cat out at this dose.
