I didn't know I had a 'problem cat' until I read that article. Don't think I will tell Pinky though.

First of all I take solace in the fact that it says "most cats are eventually controlled on 1-3 u BID of glargine,
even if they required a dose a high as 5-6 u BID in the first 1-3 months to control blood glucose."

Secondly, she is a huge water drinker and has the poor coat condition and lacks energy. I will have the other possible conditions checked out if I don't see improved glycaemic response once I return from my trip. Though it does say it may take 5 to 6 u twice daily for some cats.

interesting article - i think it's key to note that a lot of the discussion is around using insulins other than glargine (Lantus). The first paragraph on the second page is all about what we call a bounce - the cat hits lower BG numbers than its used to and its body released counter-regulatory hormones that shoot its BG high.

i didn't see anything in there that wasn't consistent with the protocol we follow. Don't really blink up to doses of 5-6units per cat, if they go much above that, test for iaa or acromegaly. most cats are controlled on 1-3u doses. It all seems pretty consistent with what we see here.

i did think it was interesting that the "drop the dose and start over" plan was only suggested for shorter duration insulins, not for Lantus.

was there a particular part you were interested in, elaine?
 
Hi Julie, not sure if you noticed or not but that article is written by Rand whose research is partly the basis for the protocol here so don't think you would see anything contradicting what is recommended here.

I think it intriqued me because when I read Hodgkins work and looked at data from people on that protocol based on scaling the doses (even with Lantus) I was always puzzled (and concerned) that they used such small, small doses and held them forever and I wondered why in the world I was up to such a high dose (4u) with not much control.

At least in the article, it says that just happens with some cats and in that sense the article made me feel better.
