I think you have a choice here. Try our protocol or stick with your vet's sliding scale. I know it is hard to take advice from strangers on the internet, but our methods do work with lots of cats. And clearly your vet's scale is not working.

The sliding scale is okay with some cats - except for the bounces. (Did you read the article on Rebound I linked in your last post?) My guess is that he went low last night and this morning is a bounce. If you shoot the higher dose into this number, he will likely drop low again and bounce again. It is a repeating cycle.

Several people tried to explain this in your last post and give advice. You might look at that post again.

Sliding scales can work, but it looks like your vet's scale is too radical. There's too much difference between the largest and smallest doses, and the "steps" are too big. Also, by using the scale, you are shooting at bounces. Those high numbers are not "real" in that they have only been caused by the previous dose being too high. You are having to constantly fight against not only the fact that Loki's pancreas is damaged, but also against the "liver panic" that happens automatically every time Loki drops to low numbers. If you have to keep doing that, you'll never be able to reduce his dose, and overall it looks like his doses need to be lowered.
