Lovey? Dosing for tonight 1.25 or 1.5?

Lovey was on Pro Zinc for 3 months before switching to Lantus. She was on 1.75 but the dose only brought her bg to the 190 range on the alpha trak at best. She needed a dose increase probably. I am sorry for all the posts.

Just curious if anyone has switched from PZi to lantus?


I started at Lantus at 1 unit. I am going to change tonight due to high numbers. Can I go to 1.5 now instead of 1.25? Do you h think 1.5 could have been a reasonable starting dose. With that consideration and the Lantus doses, and my current mumbers. Do you think it would be too risky or more risky that high readings for 3 days? Lovey has been high on Pro Zinc too in the last weeks. It seems to me that her .25 wouldn't really effect my numbers much. Does anyone else agree? Do you think my numbers would come down a little with .25 increase? My vet feels that 1.5 will not be enough and is fine to go to, but she initially wanted me to try 2.5???
With the depot building, is this dangerous? I could take off from work one day to test more. I could test for cycles 1-3 more extensively. I would have to work during cycle 4 and 6. and could test more extensively on cycle 5.
I could get a preshot and a +10 on the days I work?

Another possibility:
Could I give 1.25 for only 3 cycles. If the 1.25 does Ok or at least brings down BG a little I will keep for 6 cycles. But if not, I will take off Friday and then have Saturday and Sunday to give 1.5 and watch Lovey more closely. Is this possible?

I agree with everyone on that thread and on the thread in Health that an increase makes good sense. I usually side with "caution" especially with an insulin I never used. I would increase by .25u, and give it six cycles. If another increase seems to be indicated after that, then increasing again would be on the "aggressive side".
 
Thanks Carl for such a quick response. I usually give insulin at about 6:30 so I was hoping someone would reply.
 
One of the more challenging differences between Lantus and ProZinc is that Lantus dosing is based on the nadir -- not on pre-shot values. Since you've been seeing some numbers that are under 200, you would want to increase in 0.25u increments. Typically, you want to give the first dose from 5 - 7 days before changing the dose. This allows the insulin depot to form. It looks like Lovey's numbers have been coming down as the depot is establishing itself.
 
My lowest midcycle number on Lantus was 267 so far. Thank you for responding since I give insulin in just a little while.
