Louie's first home test - over 500!

Hi again,

I promised an update on my friend's cat Louie once I taught how to test. Here is my post from the other day with some introductory info:http://www.felinediabetes.com/FDMB/viewtopic.php?f=28&t=83117

Here is what new information I have.....

Louie has had curves and fructosamine tests done roughly every 6 months or so. Curves are done at the vet - my friend doesn't have the results, but remembers seeing a graph with a straight line - no significant dips!

Over the past few (three or so weeks) Louie has been 'off' drinking a LOT more water than usual, her coat is greasy and clumps of fur come out with the slightest effort. The only thing that has been changed is the introduction of canned prescription food (Hill's)

Apparently about a year or so ago, the vet mentioned that Louie may be insulin resistant, or even an acro cat, but the test is very expensive and wasn't done at that time.

Louie was at the vet in early October, routine blood work done, nothing abnormal other than the diabetes! The vet is suggesting that the insulin be increased to 10u BID - my friend is not comfortable with that! She is concerned that Louie is getting too much insulin.

I taught her how to home test tonight, Louie grumbled and groaned, but we managed to test before dinner time....we got a whopping 28.4 (about 510ish) fed Louie as usual (still on dry for the moment) about an hour and a half later, we tested again and got an even higher reading of 28.8 (518ish)

This is the plan of attack for now (input is welcomed and appreciated):
My friend is going to home test as much as possible over the next week. We discussed increasing the amount of wet food as Louie seems constantly hungry, Louie will continue on the mixed wet and dry for a little bit longer. My friend will call the vet after the week with as many numbers as possible and get advice then. My friend is off for two and a half weeks over Christmas time, so the plan is to make any dose adjustments / full removal of dry food then since she will be home more. She is very concerned about Louie having a hypo episode when she is at work. Her gut is telling her to try a reduction of 1u once she has some data and when she can be home to monitor.

I of course told her all about the message board and have encouraged her to at least lurk and read the awesome info here. I am thinking that she will sign up too, but for now I thought I would just update and act like a go between lol

The acro and IAA tests cost a lot because of shipping from Canada to US.

As it happens - the lab that runs the test is in East Lansing, Michigan. This is 237 miles from Cambridge, ON, or about a 4 hour road trip (each way).

If she could have her vet prepare the blood samples and freeze them - maybe we could arrange transportation to get the samples to the lab. (I am part of DCIN's transport team, and have personally escorted diabetic cats across the border 3 times since 2010)

The lab fees are approx $75 for both tests (recent price increase)

Good job on teaching her to home test -- that will help immensely, and she will see the proof whether her kitty needs more insulin or not.

Also - do have her think about switching to Levemir if her kitty does need the insulin after diet change -- Lantus can sting at the higher doses.

The High Dose moms / dads can help her even if she cannot afford to have the acro & iaa tests done. If she is home testing and posting bg numbers online - that is a big first step in the larger FDMB community being able to help her.
 
Good for you in helping her out!

The doses you describe do seem to suggest acro or IAA.

You might let her know that once she is feeding low carb and eliminated all dry, if the doses still need to be high for control, it is likely one of those conditions, so it might be OK to follow the protocol even without a test. They label it 'empirical therapy'.
