My Sugar cat Bast is still OTJ! (YEAH) However my mom's cat Lilly was just diagnosed. She had a vet appointment yesterday and the day before I did a quick ear test and got 390. So she had an idea it was coming.

Her vet says to start with switching completely to wet food before starting Lantus. This has thrown me a little because Bast started out in ketoacidosis and spent the first 5 days at the vets getting better. Then he came home and I started Lantus and wet food.

So- mom is cutting off the dry food bowl (she fed 1/2 & 1/2) and changing to low carb food. She was told to do it a week and see how Lilly is doing.

What do you all think? I have no problem teaching her how to test and such but she is afraid to push for Lantus yet- she wants to wait the week. I think she is overwhelmed. He other cat is having thyroid issues.

With a 390 and not other obvious problems I would first try a lo-carb canned diet. Then after a week start insulin if required. Better would be to test at home during that time and see if BGs are going down. If going up I would start insulin then
 
A change to a low carb diet may reduce the glucose levels from 100 -200 mg/dL, which would get him to borderline high. He may need some insulin at that point, or his pancreas may have a chance to catch up.
Have her feed mini-meals to spread out the workload for the pancreas; this may help it recover.
Add some water to the food to provide volume and hydration (diabetics will pee out some of the glucose).
 
I did a spot test today- she had not given them food yet and Lilly was 319. that is after 2 1/2 days without dry and on low carb wet. So I will keep checking for the week.
just a different start than what I did for Bast.
 
sorry It has been a mess- mom left to go take care of my gramma in another state. The 295 was 4 hours after feeding. Today we did another spot that was at least 4 hours (not sure if my sis fed her in the afternoon a spoon or two of wet food snack) and it was 235
