? recent DKA, dose advice | Loki and Alessia

Hi everyone,
My name is Alessia and I live in Italy. My cat, Loki, is a 5-year-old neutered male. He weighed 7.5 kg at the time of his diabetes diagnosis in February 2026, when he started treatment with Senvelgo.
Since February, despite eating well, he continued to lose weight. Between the end of February and the end of July, he lost an average of about 200 g per month. Before he was hospitalized, he weighed only 6 kg. He was eating two cans of wet food every day (Lily's Kitchen and Almo Nature Daily), plus a small amount of dry food, even though he has never really liked kibble.

Unfortunately, on July 31st he was hospitalized with diabetic ketoacidosis (DKA), which was caused by the medication. Surprisingly, when we brought him to the hospital, he was still active and had eaten one can of wet food that day. However, his blood ketones were 6.6 mmol/L, and he had a fever of 40.3°C (104.5°F). The veterinary team immediately started him on IV fluids, insulin, and antibiotics.

Thankfully, he has improved a lot. His ketones have now dropped to 1.2 mmol/L, his fever is gone, and all of his bloodwork has remained normal throughout his hospitalization. Since Monday, they have started him on ProZinc at a dose of 1.5 units. He will probably be discharged tomorrow, and the veterinary team will place a glucose sensor that lasts 15 days to monitor his blood glucose levels.

I'm really worried because I don't know anything about managing diabetes with insulin. I'm wondering whether the insulin dose is adjusted based on the glucose readings or if it's usually a fixed dose. If it needs to be adjusted, how is that typically done?

I know my veterinarian will explain everything to me tomorrow before Loki comes home, but I was hoping to hear from those of you with experience so I can better understand what to expect. Any tips about living with a diabetic cat on ProZinc, using a glucose sensor, or what to expect during the first few weeks would be greatly appreciated.

Hi Alessia welcome! We have so many beautiful black kitties on this forum (including my Moonie) so I’m happy to see handsome Loki here :)

You should wait for a more experienced member to provide advice, but in the meantime, if you adjust your title, you’ll likely get a faster response. There is a dropdown by your title and I’d put “?” and I would include something like (recent DKA, dose advice) in the title itself. That way, experienced members know to prioritize your thread for a more immediate response.
