Kidney and pancreatitis issues - please help!

Hi guys!

So my 16 year old cat (Jack) fell out of remission in May - he had been in remission since April 2019. He's back in remission (took me 10 days, phew) but now he's having issues with his kidney and pancreas.When we did his blood work back on May 17th, his urea was high and some of his liver results were off. They assumed it was because of the diabetes since the creatinine, phosphorus and ALT were perfectly mid-range. His electrolytes were also perfect except chloride was a bit low. They said it looked like his pancreas was a bit off from the diabetes.

Yesterday I brought him back to the vet because he was still drinking and peeing more than normal. He also lost a bit of weight (went from 13.5 to 12.5 lbs) but I've been feeding him for 12lbs for the last few years since he's overweight so weight is always hard to assess on him. He weighed 16lbs in 2021 when I started working on his weight. The vet told me his liver was back to normal, but he tested positive for pancreatitis and now his creatinine was high enough to be considered stage 2 KD (along with his urea still being high).

Has this ever happened to anyone? They said it was fast and possible that its the pancreatitis causing the kidney issues since the increase in creatinine happened super fast. They want me to start on anti-nausea medication, pain killers and sub-q fluids and a kidney supplement. Not really sure why the pain meds, he's not in pain. I'm going back today for a urine test and going to ask if they can check his GFR - his SDMA was high normal and his phosphorus is mid-normal.

I switched him back to wet food in May (as soon as I saw his BG was 27, but he refused to eat the wet food for the first 4-5 weeks (tried 4 brands, every flavour they had - he's never been a fan of wet food in general) so I switched to Young Again dry with wet on the side, always available and got him eating again but then I went on vacation Aug 1st-4th, I upped the YA feedings on my feeder in case he wasn't happy with how my mother in law "presented" the wet (he's picky) and he got diarrhea the first day. So I reduced it back as soon as she messaged me (we use a timed feeder with an app) and the diarrhea stopped but now he doesn't eat as much since I got back. Like overall he's eating everything he should, just not running for the food bowl as soon as he hears it trigger so I can't tell if he's put off of it or just not feeling well (he'll eat a day's worth if I bring it to him in my hand). He got diarrhea for a day the first time we had used YA 4-5 years ago too. Yesterday to test his appetite, I mixed half a can of high carb wet food with a full can of low carb wet (4.5oz total) and he ate the whole thing in one sitting.

The whole thing seems weird because he's acting normal and I always thought pancreatitis would make it impossible to get him back into remission so quickly and have more gastro symptoms. Before she did the bloodwork, she said he was only a little bit dehydrated but hard to tell if it was just because of his age. His physical exam was unremarkable otherwise - he didn't react to pain either.

Any advice or experience would be appreciated! Thank you!
 
Well done getting him back into remission. My Sheba fell out of remission after 2 and a 1/2 years and I was never able to get her back into remission. She also had pancreatitis flares from time to time.
A cat with pancreatitis needs pain meds, and an antinausea med and often sub Q fluids. Pancreatitis is very painful and a cat is very good at hiding pain.
I would continue to check the BG daily for a while, while he is having issues with the pancreas, just to make sure they are staying in the normal range.
It is possible the kidney values are out of whack because of a bit of dehydration, so I would wait and see what they are after things settle down again. What is the kidney supplement? Not sure that is needed at the moment. I will tag @Suzanne & Darcy to see what she thinks about it all.

They recommended the Aventi Kidney Complete but I wasn't planning on starting it until I know he needs it. At the end of May, his phosphorus and electrolytes were all in the mid range of normal except his chloride was a bit low so I can't really see the calcium or phosphorus changing dramatically that quickly. Still waiting for a copy of his current blood test results so I can share it.
