Has he ever had DKA? His numbers have really shot up...are you aware of any infections he has?
DKA is caused by infection + not enough insulin + not enough food/water
Is he eating? How about fluids....is he drinking and do you ever give him subqs? I'd be sure he is getting plenty of food and water or fluids and I'd keep a close eye on the ketones. If they start to go higher at all or don't come back down with the food and fluids, I'd get him to the vet immediately. Ketones can develop quickly and also progress into DKA quickly.
Even if you get them back to negative in quick order, I'd take him in as soon as possible and see if he has a dental issue, UTI, etc...any kind of infection. His numbers are high which could also be indicative of infection.
