? Is it unusual to give 4units?

Good afternoon, We have been following the TR protocol and are now up to 4 units. There has been no change in the pattern that I can see. Is this unusual? Do you think there could be something else wrong? His numbers seem to be higher than ever.
 
It’s not unusual at all. A cat needs as much insulin as it needs. He’s new to insulin but maybe he’s been diabetic for awhile. It’s possibly glucose toxicity. There red other conditions such as Acro but he’s so new to insulin I suspect he just hasn’t hit the breakthrough dose.

Ok, thank you! I am just worried because of the higher numbers! We will just keep on truckin' then:)
 
From the fdmb glossary:

GLUCOSE TOXICITY: If a cat’s BG numbers have been remaining in a higher than desirable range, the cat’s system may adapt to the higher range making it harder for numbers to respond to insulin. The higher range becomes the cat’s new “normal.”

Basically their bodies get used to higher numbers, so you need to get to a higher dose to break through it. It's a type of insulin resistance. Can I ask why you gave 3.5 unit last night? That restarts the cycle count on this dose. Also, there was no mid cycle test the night before than. With the numbers he's in, ideally you'll not spend more than 6 cycles on a dose before increasing, if he keeps showing you nothing below 200. But you can only increase safely that often with TR as long as you are getting at least two tests per cycle.

Sorry, that was a mistake. I did give him 4 units. I will fix it.
 
If you look back to the 2020 sheet you will see that I kept giving him insulin and nothing seemed to happen. I don’t know how many days I said, “this insulin is doing nothing! Nothing!” It was a bad time. We went through every type of insulin and it made no difference. I kept trying fresh vials, etc. Of course, my sweet boy had acromegaly and I didn’t know it for a long time. Until the beginning of 2021 when I started getting the advice of this Board, by poor baby was not getting much help. Now your kitty hopefully doesn’t have acromegaly, but it is possible that you just haven’t hit upon the good dose yet. I would keep increasing every six cycles unless I saw blue numbers.
