Hi, Baby does not take Lantus based on her weight, we start low & increase/decrease as #s indicate us to. However, last time her breakthrough dose was 1.25 & she is currently on 1.0, Baby weighs 11.3lbs, if we did go by weight, what would her dose be? Thank you.

Using a weight based formula for determining a starting dose of Lantus or Levemir:

the formula is 0.25 unit per kg of the cat's ideal weight
if kitty is underweight, the formula frequently used is 0.25 unit per kg of kitty's actual weight
if the cat was previously on another insulin, the starting dose should be increased or decreased by taking prior data into consideration

We don't dose cats by weight as a general rule, we dose by BGs (nadir and preshot numbers). The peak/nadir BG is vitally important to know when adjusting doses.

However, some starting doses are determined by weight if they are following the Rand protocol : http://www.uq.edu.au/ccah/docs/diabetesinfo/link4.pdf

They suggest 0.25u/1kg of ideal weight. We tend to find that this is too high a starting dose.

Thanks! I was just curious - there is NO WAY I am basing her dosage off of weight, just curious as to what it would be ;-) you all have been helpful.
