Funny, I asked that question a couple weeks ago, and from the responses people seem to do either way. I've tried both and seems to come down to the angle that works best for you. Hope that helps!
Some do the outside and some do the inside. I do the outside because it's the easiest place for me to get to, since Calliope is not wholly cooperative.
Wherever you can get blood from is fine. My black cat, Spot, needed to be tested inside his ear because he didn't bleed well on the outside and also his skin was as black as his fur so I couldn't see well. The other 3 kitties bled better on the outside of their ears, including a second black kitty. Interestingly, I could not freehand Spot's pokes successfully either and had to use the lancing device. He had very thick ears. So all kitties are different, and if what you're doing is working, no need to change it.
Sometimes I would poke-through Smokey's ear and I would get part of the drop from
the outside and the rest from the inside if it was not enough from the top.
