tutzie is on 3 units of lantus every 12 hours this is day two I have checked her blood two days around 1130 am and it's 16.3 yesterday and 17.6 mmol today. I think I need to increase it?? Going to ask the vet tomorrow. These readings are better than Friday when she got diagnosed she was in the 30s I know when I am that hi I feel like poooooo
 
The doses of Lantus are adjusted based on the nadir, the lowest glucose between shots. This is often between +5 to +7 hours post shot, but can and does vary. Before you change the dose, please get some tests in that period.
Glucose tests at the vet may be increased from 100-180 mg/dL due to stress.
 
The Lantus dose should also be held for 5-7 days if this is the first time giving it. It can take that long to see the full effect of the dose on numbers.
 
Yes, I would keep her on the 3 units for another 4-5 days or so. Then test her both shortly before her shot AND around 6 hours after. If the test 6 hours after reads over 10-13, then try an increase of .25 (I assume you're in Australia??) and hold that increase for 4-7 days.
 
It takes 7 to 10 days to adjust to a specific insulin dose. The key to stabalizing the blood sugar is through regimentation. Testing the same time each day before a meal and subsequent insulin delivery. Feedings the same time every days as well. I only do a blood sugar curve once monthly or if her sugars skyrocket consistently. I have found that her sugars elevate relate to stress or she has been able to get to additional food. She is receiving 1 unit of Lantus twice daily and over all is doing quite well. The start was somewhat rocky last May; but, she is doing so very well since then. Good luck from Jane and Stewey
