INCONSISTENT BG NUMBERS / Cat on Prozinc

Hi! First time posting! My 5 year old, male cat, Socks, was diagnosed with DM (and DKA) in early June, so has been on insulin for 3 months now. He was hospitalized for the DKA for 2 days and bounced back well. It was recommended by my vet after diagnosis to start him on the prescription hills M/D canned diet, but after weeks of high BG readings (using home tester), and a large insulin dose (Prozinc) at 5u BID, I went against vet orders and switched him to Merrick's canned Pate (2 cans/day - 1 can every 12 hrs w/ shot)

He has been doing OK (not well regulated) with the merrick food, and has been on 3u BID for a few weeks now. The issue is, we will check his lowest point, and it could be around 100, and 6 hours later check his highest point (before shot and food) - and it can be as high as 500... so within 6 hours, his BG's shoot up like crazy (sometimes!)

The reason for my post is last night, his highest point was 250 (awesome), but about 6 hours after his 3u shot, he was only at 175, which is ok - but just last week, we've seen dramatic drops in his insulin, as he went from 450 to 150 in that 6 hour mark. I was terrified to give him his 3u at 250 as I know he had a 300 point drop from the same dose days prior, but I checked his BG 6 hours in, and he was ok.... but it doesnt make any sense!

Has anyone had a problem regulating their cat using Merrick canned food? The only thing I can pinpoint is the consistency in carbs, or how they are breaking down... I have crossed everything else off the list such as injection location, the insulin is fine, etc. I just cannot seem to get him to be around the same levels even day-to-day.

He was technically regulated on the hills m/d, as his highs were only around 300/400, BUT lows would ONLY ever get to around 200 so I was not happy with that.. But after this switch to Merrick, his high numbers have just been ALL over the place - but his lowest BG numbers (6 hours in) are consistently good, around 100...

The vet reccomended he may need to have shots 3 times/day, so every 8 hours, but there is no way I could ever do that as my work day is 8 hours long, taking 1 hour each way for travel.

ANY help or suggestions would be so appreciated...
 
5 units bd is a big start dose! 1 unit bd is more usual, even though you were sensible in dropping his dose when switching his food he is dropping fast. Prozinc tends to give quite a gentle drop, and I suspect he needs less that 3 units. Can you add a signature - important as he has had DKA, and start up a spreadsheet so we can see what his numbers have been doing, the experienced users can then have a look and be able to advise better. I am not in the US so not familiar with the various foods but I think Merricks is quite low carbs. I suspect his dose is still too high and needs to come down but it would be useful to see the numbers. Apologies, my cat was started on 1u, increased to 2u after 3 days - then maintained at 2u for 1 week, and increased 1u each week until getting to 5u - so was at about 5u after 1 month of diagnosis. But after switching from the rx M/D to Merrick, his numbers were immediately lower so we tapered the doses down to 3 over about a week. We've checked him thoroughly to rule our any Somogyi effect - but that is a great suggestion. I will do that tonight! Thank you for your reply!
 
Which Merrick foods are you feeding? Many are low carb, but not all. (We're working on a new food list and are researching Merrick now)Also since insulin is a hormone and many things affect how it is used in the body, each day can see variations in numbers. That is why we stress testing so much. When you get that spreadsheet(SS) going, we will be better to help you.
 
Gotcha gotcha... He is currently on Merrick Puurfect Bistro Grain Free Chicken Pate. I emailed Merrick before switching to their brand, and they confirmed with me that this food only had a NFE 2.59% - which was a lot lower than other flavors so I've stuck with this for now... Can't wait to get home to work on my spreadsheet!

Glad to see you have posted on this forum. Here is what I wrote on your post on the Health forum:

I hope you will take up Marje up on getting your spreadsheet going. Even seeing a few numbers - especially those midcycle numbers - can help us see what might be going on. The encouraging thing is that he does go lower; we just need to figure out how to use the insulin effectively so he is on a more even keel.

The food sounds like a good choice. How much does he get? One idea is to add some warm water to the food so it seems like more for him to eat. You can give him food more often if you think it will work better - often people divide the amount of food up into 4 meals per cycle. It may also be that he is not yet in regulated numbers and so his body doesn't use the food effectively - that can make him hungry more often.
 
Hi Justyn and welcome to the Prozinc forum
It can take a really long time for cats to get regulated (Murphy has been diabetic for 13 months and still isn't) - so 3 months is still very early - no need to worry
Once your spreadsheet is set up, it will be easier to get a feel for Sock's response to insulin.
 
Awwww wow! Thanks for that tip, I really thought things should be more normal by now so that's a relief... I don't get off work for another 4 hours so I'm aiming at getting it done tomorrow!
