Hi Heather,

It would really help us (and you) if you could get some mid cycle numbers. He is getting decent numbers, but I wonder how low he goes midcycle. (You did get some nice nadirs early on, but we don't know now what is going on.) Can you do a curve so we can see what the PZI is doing? It is possible that he goes low and bounces back up at ps. Or he could be high and flat through the whole curve.

Your SS notes he may have gotten into dry food. Is it around here he could get into it on a regular basis? If so, that could certainly be part of the problem.

About testing - Do you heat his ear each time? That made a huge difference for us. What size lancet are you using? Sometimes a lancet that is 25-27 guage will make a bigger hole. And you can always double poke - just make a second poke in the same spot as the first. Sometimes a tiny thin smear of vaseline will help the blood bead up.

Welcome to PZI land. I hope we can help you figure out what is going on with Giz.

Hi Heather!

Curious why the vet had you go from 1u to 2u back on the 26th? We usually recommend people move in 0.5u increments, or even 0.25u depending on your numbers. Otherwise it is easy to miss the right dose.

You had a nadir on the low side on 2/27 on 1.5u. From there I would probably have tried a dose of 1.25, and if the #s weren't good enough on that, maybe something like 1.4. The picture is a little confusing the past few days with the dose changes, so I would try to pick one dose and stick with it (maybe 1.25 or 1.5) for a couple days, and try to get more mid-cycle data like Sue recommended.

I definitely would not even consider jumping up to 3u like the vet suggested. It's not clear to me without more mid-cycle data if the 2u is too much, so I wouldn't want to raise at all from there w/o getting more data. If you do determine a dose increase is warranted (based on PS and nadir data, not on PS data alone), then I would move in 0.25 or 0.5 increments.

Hope that helps! Keep asking whatever questions you have, and we will try to help you as best we can!

If you have any emergenies, be sure to post on Health, as traffic can often be slow here in PZI.

We actually just discovered the vaseline on the ear works wonders! This will make testing so much easier for all of us, so we'll get the curve done soon and post that. What is an ideal range for Gizmo to be in? No, he does not have access to dry food on a normal basis, but i forgot to put it away after my other cats were done eating and I saw Gizmo walking around it.

Although ECID, we generally say a kitty off insulin ranges between 40 - 120 with the majority of time in double digits. A cat is considered regulated on insulin when he is in the 200s at pre shot and 100s at nadir.

Glad the testing is getting easier. We'll be watching for your numbers.

Just wanted to say hello and welcome. So glad to hear the vaseline helped you and Giz. I use the vaseline trick too. My cat Max is a fluffy a cat and I've literally seen his fur soak up a perfect drop of blood right before my eyes without the vaseline. You've come to a great place for help. The advice I received here really helped me with Max. Keep us posted on the testing and #'s.
