Chloe has suddenly had a fall out of her 1.5 year remission and I feel overwhelmed at what I’m doing.
I have her spreadsheet updated. I’ve been doing SLGS but wondering should I switch to TR?? her readings aren’t really improving and it’s been 6 days. Is that usual? My fear is that she’s going to get ketones or DKA if I can’t get her controlled fast enough. How long is the usual initial starting time to get controlled or see Lantus start working. Please help us :( scared seeing her like that and worry about her body taking all this.

Mary -

One of the reasons that a cat can fall out of remission is if there's an inflammation or infection present. The need for a dental is probably the #1 reason this happens. I would get Chloe in to see your vet for some labs and a thorough check-up.

As for your question about "how long" -- if you recall, it takes about 5 - 7 days for the Lantus depot to be built up. You may not see much for several days. (Remember people telling you, "It's a marathon, not a sprint!") It doesn't matter if you decide to follow SLGS or TR with respect to how long it takes to see the effect of insulin. With a depot-type of insulin, you have to wait out the depot being formed.

Hi Mary…in the same boat here, back on the juice as of yesterday. Shocking for me was how quickly this happens and worrying about an underlying cause. It is definitely overwhelming and scary.

Hi Mary,
If you are concerned about ketones and DKA I would suggest you buy some ketostix and test the urine for ketones.
Also make sure Chloe is eating well.
Don't forget to get tests in during the pm cycle as well as the am cycle.
If she is not having any dry food and you can test in the pm cycle as well, I think it would be a good idea to swap to TR.
With TR you will need to test before every dose and again at least once every cycle.

Hi, thanks fir your info. Chloe has her teeth checked just recently so that’s clear. Also had full labs which should be in her SS. I’ll keep being patient and see how it goes.

Hi thanks for your info. She gets only fancy feast pate and freeze dried chicken, so I can switch to TR. she eats well, no issues there. I do have some ketone strips I used before so I can try again. She usually only pees once a day or maybe two, but that’s normal for her. No change there.
I can check am and pm not a problem. What do you mean by once every cycle?
I'm happy to do whatever it takes to get her controlled again!

:bighug:
To do TR you need to be testing at least 4 times a day.
So the 2preshot BGs and then at least 1 test during each of the two cycles in a day.

Ok thank you for explaining! I will start 4 tests today and tomorrow. I’ll be home bc I have sinus infection and upper respiratory infection:arghh:
Her +6 was 250. I’ll check again before dinner.
I usually feed her twice a day and she eats a full 3oz can each time plus grazes on freeze dried chicken (no carbs). Should I feed her a snack of 2 tablespoons FF at all throughout the day or before bed? It’s been a long time since we did this so I forgot sorry. Thanks for helping us.:bighug:

I would offer a couple of snacks during each cycle of a teaspoon each snack and see how that goes.
You don't want to make her overweight so if she doesn't need to put on any weight, you could use some of the main meal food for the snacks or if you are happy to feed a bit more, add the snacks into the daily allocation of food.

Ok :cat:thank you!
I gave her a little bit of her FF. She had lost 1/2 lb since having problems with controlling her numbers but I don’t want her gaining too much bc that’s no good for her arthritis :( when she is 13-13.5 lbs it’s a good level!

Re. the ketone strips, please make sure they are not past their expiration date. The ketone strips tend to be more sensitive to the age of the strip given that the readings can be affected by humidity, storage conditions, etc.
