Hypothetical question about late injection

A little background info...I am a respiratory therapist that works 12 hour night shifts at a hospital. I am also a member of an air ambulance team. What this means is that some nights I go to work at 6pm but don’t get to come home in 12 hours. I can be dispatched for a flight and not get home for several hours after my regular time off.

It hasn’t happened since Ben got diagnosed but I know my luck won’t hold. So I want to be prepared. What should I do if I can’t get home at his regular injection time? I have a friend that is willing to come give him his insulin but I don’t think he’d let her check his sugar. Should I have her shoot a smaller dose without checking or skip that dose until I can get home?

Hi Lisa, have you considered having your friend come do /learn the testing while you're there? That would be the best case scenario, since your kitty might be more amenable if mama is nearby for comfort.

So early in your insulin use, with no signs of regulation yet, it looks like it could be catastrophic to dose with no input (to me), I would think it probably safer to skip if it happens at this point.

*disclaimer* I'm not a pro, but to me it feels like there is a lot of bouncing and several as low as you ever want to go moments in there, I would personally probably switch to a start low go slow approach, where you don't change the dose for several cycles, and see if that helps some (I'd start really low, like maybe .5 or .6 then go up -or down- after a few cycles if needed) It might help convince his body that down is the right direction and slow down the panic/bouncing. <just my thoughts>
 
Since you are using ProZinc yo can just reduce a dose if the time before the next scheduled dose is like less than 9 hours. With the greater than 12 hours do not be tempted to give a higher dose when the time to next shot would be substantially reduced.
A few years ago on a weeks ends I was on an 8/16 hour cycle and even used Lantus

I would not suggest doing this with a new diabetic who is not at all regulated. Some people will data will shoot early, if the number is definitely rising and over 200.

As Barb suggested, if you could teach someone to test, that would be best. If you are close (say +13) you could shoot IF you can adjust the dose the next cycle and push it forward an hour. Or you could shoot a smaller dose than usual and then see what he looks like the next cycle and try to adjust there.

IMHO, ProZinc is quite adjustable once you have data. Until then you can be flexible if you have the freedom to shoot later and adjust back on a 12/12 schedule that will work for you next cycle. I don't with your schedule if you have those options.

If all else fails, skipping is just like giving a fur shot. It happens and you adjust later.
