How does a vet differentiate between a UTI & polyonephritis?

Are there different tests to determine where the infection actually IS? Besides the urine/bacteria culture & bloodwork?

Per the Merck Manual, 10th edition
"Animals with acute pyelonephritis exhibit kidney or flank pain, fever, malaise, and sometimes vomiting, polyuria, and polydipsia."
