hours late for shot and bg is still low

I am not sure if this means I need to reduce his dose again but as of 6pm eastern he is 4.5 hours late for his shot and he is still at 115. I know that is too low to give him any insulin and I am not exactly sure what to do.
 
What insulin are you using? I would also add the insulin type to your spreadsheet like in the Remarks column at the top only.
What was this morning's preshoot and dose?
 
I am going to disappear for a while but as you monitor, and if numbers get up high enough to shoot.... remember that the next shot time is 12 hours after that.
How important is the schedule you are on? You need to consider that.


You aren't getting any night time tests and that would really help.
Many cats tend to go lower in the pm cycle so this dose might be really good for Cuddles but it's hard to know for sure without more data.


Keep watching... let's see if he goes up by shot time.
 
There are basically 3 options if your cat is too low to shoot.

1. Stall. Don't feed, don't shoot any insulin, test again in 20-30 minutes to see if the number is rising or falling. Post here for help.
2. Skip. Expect to see higher numbers in the next cycle, as the insulin depot will have drained a bit.
3. Reduce to a token dose, like half the normal amount.

Which of these 3 choices you pick depends on a lot of factors, such as how much test data you have on your cat, what your schedule is like, will you be around to monitor in case your cat drops below 50, do you have your hypo toolkit ready and waiting?

Cuddles went from 85 to 115 in 4.5 hours, but that was with some food on board. With the limited amount of test data that you have, I think you made a good decision to skip the shot. I would not reduce in the next cycle, IMHO. With the skipped shot, the depot will drain a bit and you need to refill that depot. Of course, your cat may have other ideas, and give you more really low numbers in the next cycle.

You might want to read this post, from over in the Lantus TR forum, on Shooting and Handling Low Numbers and being data ready. One test that would really help you out, is to get a regular "one last test before you head off to sleep" test. Many cats drop lower at night so this night test can provide us with some very useful data.
 
Yes I meant +13 and +17.5. I just tested him now at +19 and he is now at 154. Thanks for all the advice.
