Hi! Question about feeding multiple meals and an accurate BG reading! Help!

Hi everyone! I'm Paula and I have a black cat named Sammy who has had a run of bad luck, his most recent being (after he had to have a rear leg removed) to pancreatitis, and now diabetes. He also has asthma so he was on steroids, which probably had a lot to do with his acquiring diabetes! (He is not longer taking them). He lost a lot of weight and I almost lost him, but he finally came home. The vet tried to talk me into feeding him dry food all day long on top of regular meals, but I only feed low low carb wet diet (this isn't my first rodeo with DM haha). So, he's been eating 3-4 meals a day but when I take BG tests his sugar is still high, which I suspect comes from him eating quite a lot to catch up on his weight. My vet seems reluctant or too busy to return my many requests for communication, and I have turned to you all, with the understanding you are my support staff. I appreciate all your help and advice, and many thanks to all, in advance.

Just a few questions first so we can gather some info about Sammy.
When was he diagnosed? What dose was he started on?
What insulin are you giving and what dose now?
What glucose meter are you using?
Do you have any BG data to share with us?
Would you consider setting up a spreadsheet and home testing the BG and sharing with us?
We are very data driven here and need to see BG data to be able to help you. We are here 24/7 if you need help.
I doubt it is the frequent snacks you are giving him, especially if he is underweight. Just don’t feed for the 2 hours presto as we don’t want the pre shot BG food influenced. And it is generally better to feed in the first half of the cycles. We recommend feeding a meal before all the insulin doses and then a couple of snacks during both the am an pm cycles each day.
 
Sammy had his leg removed in July. Somehow got pancreatitis and was diagnosed with DM about 3 weeks ago. He was started on vetsulin which was upped to 2.5 units after a fasted BG at the vet, where it was in the 600s. I have an Alpha Trak and new test strips. I've had a few diabetic cats throughout my life, and am a vet tech, so I'm fairly competent about doing the BG curve although Sam is not quite the happiest with ear sticks, he's been getting better. I've been doing a 6 hour post dose just to see how he was doing and also did a 4 hour today and an 8 hour today. His numbers continue to be in the upper 300s and 400s. However, I confess to feeding him at least three - four wet food dishes a day, and he has freeze dried (moistened) Stella and Chewy overnight (I do have three cats though, so not sure how much he's eating himself so far). I'm guessing that as the vet mentioned he should be continually fed (he mentioned dry DM food! I said oh heck no!) as he was so skinny prior to the pancreatitis being eliminated. I guess his meals, now that he's gained a good pound or two back, while very very low carb, should be more consistent (he's excited to eat every time I enter the house, and since I work walking dogs and doing horses am constantly in and out of the house). I do think this larger amount of eating MUST be affecting the BG, especially as, ignorant me, I fed him less than an hour before the mid dose BG. I will endeavor to immediately stabilize his eating schedule and keep the extra meals replace by small snacks earlier in the cycles. He does always get fed before his am and pm dose to make sure he has food at least 3 oz of wet low carb or raw food prior to his insulin. Once I get a bit better with him and the ear sticks (he has black ears and the most incredibly small ear veins, unlike my previous sugar babes), I will work on that spreadsheet and share it with you. I am also trying to get alternatives to deal with his asthma, as steroids are out. Again, the vet's not being helpful here, but I do have some leftover albumin and an aerokat for emergencies. I am also going to try to switch to prozinc, as even tho it costs more I had good results with it in the recent cats. (Same practice but a different doctor - she left the practice for her own since my last S.B.). Thank you again for all your help. I am quite attached to this boy and feel that between being an outdoor cat in his 4 legged days and all the stress from the steroids that this could maybe have been avoided? I don't know, but it's not a good feeling. He deserves the best I can do for him. As they all do!
 
I think swapping to either Prozinc or lantus is an excellent idea. Vetsulin does not last the 12 hours and you will find the pre shot Bg will always he high again.
You can get Lantus from Canada quite cheaply or generic brands more cheaply as well.

Lantau and prozinc dosing is based on the nadir. It the preshot.
Are you taking BG tests around +4 with vetsulin? The BG could be dropping very low and then going back up to the 600s with vetsulin and you would not know u less you are testing mid cycle. Increasing with this scenario would be dangerous.

It is fine to feed him anytime during the cycle as far as tests go. It is only the 2 hours preshot that you need to not feed. Cats need to eat during the cycles and the testing reflects what is happening during the cycles when food is given. That’s what we need to see.

There is no need to wait for you getting better at testing before setting up the SS. We can help you with that is you like…just ask.
We have cats here with asthma that need steroids. Asthma needs to be kept under control. We just work around the steroid and the dose if it’s needed.
 
Thank you so much! The vet, instead of calling me, wants Sammy dropped off Monday morning prior to feeding or insulin, probably to do a BG curve? Or maybe just a preshot test? I wasn't even informed exactly why. It's very annoying. He's after the money, instead of communicating with me and answering a few simple but important questions. Not like I haven't put a new wing on his practice with all my cats! haha. And, yes, the earliest BG was +4. Others were +6. I figured middose, not having had much experience with vetsulin. +4 were even higher than +6 so I don't know WHAT'S going on. Need to do a curve or, if the vet is going to do a whole day, demand a copy for starting the spreadsheet here.
